摘  要:目的探讨颞叶癫痫(TLE)患者中有海马硬化(TLE+HS)和无海马硬化(TLE-HS)丘脑扩散张量成像(DTI)的变化,揭示癫痫网络的丘脑DTl改变及其临床相关情况。方法选取10例TLE+HS和10例TLE—HS患者,正常对照组10例纳入本研究。使用GE3.0TSignaHDxt超导MR扫描仪行DTI数据采集。分别计算表观扩散系数(ADC)及部分各向异性(FA)图,观察TLE+HS、TLE—HS及正常人双侧丘脑ADC值及FA值的变化。结果TLE+HS组、TLE—HS组与正常人相比ADc值和FA值差异均有统计学意义(P〈O.05),患侧丘脑FA值低于对照组,患侧丘脑ADC值高于对照组;TLE+HS组与TI,E—HS组双侧丘脑的FA值和ADC值比较差异有统计学意义(P〈O.05),患侧丘脑的FA值低于对侧,患侧丘脑ADC值高于对侧。结论颞叶癫痫中双侧丘脑的DTI参数有着特殊的改变,DTI技术的敏感性对癫痫的定侧以及颞叶以外其他部位的细微结构变化具有一定的应用价值。Objective To explore the diffusion tensor imaging{DTI} changes of the thalamic in the temporal lobe epilepsy (TLE} patients with hippocampal sclerosis (TLE+ HS} and without hippoeampal sclerosis {TLE--HS} , and reveal the DTI changes of the thalamic in the epilepsy network and its clinical correlation. Methods Three groups including 10 patients with TLE+ HS, 10 pa- tients with TLE--HS and 10 healthy volunteers (controls) were selected. DTI data were acquired by GE 3.0T Signa HDxt super- conducting MR scanner. The apparent diffusion coefficient (ADC) and fractional anisotropy {FA) maps were obtained,the changes of ADC value and FA value for bilateral thalamic of TLE-k HS, TIRE-- HS and controls groups were observed. Results There were statistically differences (P〈0.05} for the ADC and FA values between the TLE+HS group, TLE--HS group and controls group, The thalamus FA value on disease groups was lower than that on the control group, and the hypothalamus ADC values on disease groups were higher than that on the control group. There were statistically differences (P〈0. 05) for the FA value and ADC values in the bilateral thalamus between TLE+ HS group compared and TLE--HS group, FA value on the disease hypothalamus was lower than that on the contralateral side, and ADC values on the disease hypothalamus were higher than that on the controlateral side. Conclusion The bilateral thalamus of the temporal lobe epilepsy have a special change in DTI parameters. The sensitivity of the DTI technique has an application value for lateralizing the epilepsy and showing subtle structural changes in extra-temporal region .
