肝动脉栓塞联合射频消融对兔肝癌模型细胞凋亡的影响

摘　　要：目的探讨肝动脉栓塞（TAE）联合射频消融（RFA）治疗后的兔肝Vx2肿瘤细胞凋亡情况。方法cT引导下经皮穿刺建立45只新西兰大白兔VX2肝癌模型，3周后每只瘤兔肝肿瘤直径为2．5～2．7cm。随机分成对照组（假性治疗组）、单纯射频组和肝动脉栓塞7d后射频组共3组。每组于治疗后1d、4d、7d各处死5只瘤兔，切取肿瘤中心组织及边缘组织应用免疫组化检测肿瘤细胞凋亡表达。所有数据均用统计软件包SPSSl3．0处理。计量资料用；±S表示，以P〈0．05有统计学差异。结果单纯射频及联合治疗组消融区周边的肿瘤细胞凋亡指数（AI）明显高于对照组（P％0．01），以射频后1d最为明显，后逐渐减少，7d后虽明显减少但仍高于对照组。结论单纯射频和射频消融联合肝动脉栓塞治疗兔肝VX2肿瘤后1周内均能诱导肿瘤细胞凋亡。与单纯射频相比，联合治疗能诱导更多的肿瘤细胞凋亡。Objective To study the cell apoptosis in rabbit hepatic cancer models after transcatheter arterial embolization {TAE） combined with radiofrequency ablation （RFA）. Methods Forty-five New Zealand white rabbits were implanted percutaneously with VX2 tumor cells in the liver under CT guidance successfully. Three weeks later, there was a focal tumor （2.5--2.7 cm） in the liver of each rabbit. They were divided into 3 groups at random： control group （pseudo-therapy group）, RFA group and combined group with TAE and following RFA 7 days later. Apoptosis of tumor cells was detected in three time point at day 1, 4 and 7 after different treatments in three groups. Five rabbits of each group were sacrificed at each time point. The peripherial tumor and central tissue of the ablation area were examined pathologically by immunohistochemical methods to detect the tumor cell apoptosis. All the data were analyzed by SPSS 13.0 software. Quantitative data were showed by ~~s, and P^0.05 was defined as a threshold for statisti- cal significance. Results Apoptosis index （AI） in the margin of the ablation area in RFA group and combined one were higher sig- nificantly than that in control （P〈0.01）. They reached their peaks at day 1, and decreased gradually along with the time to a low level at day 7 which were still higher than that in the control. Conclusion Both RFA and RFA combined with TAE can induce tumor cells to apoptosis in 1 week after treatment9 and the latter results in more cells apoptosis.
