摘　　要：目的介绍内荷包缝合法治疗在儿童交通型腘窝囊肿中的应用及疗效。方法2008年5月至2012年3月我院收治26例交通型腘窝囊肿患儿，男16例，女10例，年龄6．2～11．6岁，平均年龄8．3岁。3例为复发病例，其余为初发病例。术前体检及B型超声诊断明确为交通型腘窝囊肿，术中采用打开囊肿、内荷包缝合腘窝囊肿基底部的方法缝合囊肿与膝关节相通处关节囊缺损。术后不使用抗生素，下肢长腿石膏托固定3周，石膏拆除后开始患肢功能锻炼，逐步恢复正常活动。随访囊肿复发及膝关节功能情况。结果所有病例均采用内荷包缝合的方法，手术时间22-29min，平均26min。术中无周围血管神经副损伤，术后随访1．0～4．8年，平均3．4年，术后切口均愈合良好，无感染，无畸形，无膝关节疼痛，无复发病例。结论内荷包缝合法治疗儿童腘窝囊肿安全有效、复发率低，操作简单，值得推广。Objective To introduce internal purse-string suture in the management of popliteal cyst in children and evaluate the clinical outcomes. Methods Between May 2008 and March 2012,26 children （16 boys, 10 girls） with a mean age of 8.3 years （ranging 6. 2 to 11.6 years） were treated for popliteal cysts with internal purse-string suture in our hospital. Among them, 3 patients were with recurrent cysts, while the others were with primary cysts. Preoperative physical examination and B-ultrasonography confirmed the diagnosis. The cysts were opened in advance and internal purse-string suture were used to close the channels between cysts and knee ioints. No antibiotics were used. Functional exercise of the affected lower extremities started after fixed by cast for 3 weeks, and the patients returned to full activity gradually. Cyst recurrence and function of the knee joint were followed up. Results All cases were treated by internal purse-string suture. The mean operation time was 26 rain （range：22-29 min）. No injuries of major nerve and vessel were encountered. Cosmetic appearance and knee function were satisfactory after followed up for an average of 3. 4 years （range, 1 to 4. 8 years）. No recurrence was observed. Conclusions Internal purse-string suture appears to be a safe, effective and simple techniques in managing popliteal cysts in children with lower recurrence rate.
