肝胰十二指肠器官簇移植术后并发精神异常的原因分析及护理

摘　　要：目的探讨肝胰十二指肠器官簇移植术后患者早期并发精神异常的原因及护理体会。方法回顾性分析2009年3月至2011年8月中山大学附属第一医院收治的5例终末期肝病合并糖尿病行保留受体胰腺的肝胰十二指肠器官簇移植术患者的临床资料。结果 5例患者术后早期均存在不同程度的精神异常,其中焦虑3例、抑郁2例、睡眠障碍5例,经对症处理及加强护理后均好转。结论肝胰十二指肠器官簇移植术后早期精神异常的发生与多种因素有关,加强围术期患者精神状态的观察和护理有利于患者康复。Objective To investigate nursing experiences and causes of insanity after combined ＇en bloc＇ liver and pancreas transplantation in early stage. Methods Clinical data and nursing experiences of insanity in 6 cases developed end-stage liver cirrhosis complicated with insulin-dependent type 2 diabetes,who performed combined ＇en bloc＇ liver and pancreas transplantation, were retrospectively analyzed from March 2009 to August 2011. Results All of five patients complicated with syndrome of insanity in early stage after transplantation,and 3 of the; were delusional type,the other 2 cases were depressive. All of them had dyssomnia. After treatment and careful nursing,all of 5 cases recovered. Conclusion There are several reasons contributed to the patient＇ insanity after combined ＇en bloc＇ liver and pancreas transplantation,and it is beneficial for recovery of those patients via nursing and observation of mental state of the patient in perioperative period.
