摘 要:目的:分析老年患者采用鼻塞给氧联合高氧液对术中动脉氧含量的影响。方法:选择我院20i1年6月-2013年11月60例硬膜外麻醉的老年患者,按病人住院号的单双随机分为两组,单号为对照纽,双号为联合纽。对照组30例患者采用常规鼻塞给氧,联合纽30例患者在对照纽基础上给予高氧液静脉输入,观察麻醉前、麻醉后15、30、45、60min动脉氧分压(PaO2)及动脉血氧饱和度(Sa0。)变化。并记录两组患者术中动脉血氧指标发生情况。结果:两组患者麻醉穿刺前Pa02、SaO2水平比较差异无显著性(P〉0.05)。麻醉穿刺后15min、30min、45min、60minPaO2、SaO2水平两组均明显上升,但联合纽上升幅度明显高于对照纽,纽间比较差异具有显著性(P〈0.01)。联合纽低氧血症发生率明显低于对照纽,纽间比较差异具有显著性(P〈0.05)。结论:高氧液静脉输入是一种全新的氧供途径,可改善老年外科手术患者硬膜外麻醉术中动脉血氧量,从而大大降低了术后低氧血症的发生率。Objective: The objective of the ;resent studywas to investigate the effects of 2asal oxygen combined with high liquid oxygen on the arterial oxygen contents of the elderly patients during operation. Methods: Sixty cases of epidural anesthesia elderly patients were enrolled in our hospital from June of the year 2011 to November of the year 2013. The patients were randomly divided into two groups according to the single and double admission numbers of the patients: The single admission number was the control group and the double admission number was the combination group. The 30 cases of patients in the control group received routine nasal oxygen,while the 30 patients in the combination group received routine nasal oxygen combined with high fluid oxygen infusion. The changes of arterial oxygen pressure (PaO2)and the saturation of arterial blood oxygen (SaO2)before anesthesia and 15 rain, 30 rain, 45 min and 60 rain after anesthesia were observed. Arterial oxygen indite of the patients in the two groups during operations were recorded. Results: There was no sig- nificant difference in the levels of PaO2 and SaO2 of the patients in the two groups before anesthesia and puncture (P〉0. 05). The levels of PaO2 and SaO2 15min,30min,45min, 60min after anesthesia and puncture in the two groups increased obviously, but the increased extents of PaOz and SaO2 in the combina- tion group were higher than that of the control group, and the differences between the two groups were significant (P 〈 0.01). The incidence of hypoxemia in the combination group was significantly lower than that of the control group,and the differences between the two groups were dramatic (P〈0. 05). Con- clusion: High fluid oxygen infusion is a complete new way of oxygen supply,and can improve the arterial blood oxygen contents of the elderly patients with surgical operations during the epidural anesthesia operations, thus greatly reducing the incidence of postoperative hypoxemia.
