相同剂量3%盐酸氯普鲁卡因与2%盐酸利多卡因用于剖宫产手术麻醉效果的比较

摘　　要：目的:比较相同剂量3%盐酸氯普鲁卡因,2%碳酸利多卡因连续硬膜外麻醉用于剖宫产手术的麻醉效果。方法:剖宫产手术患者180例,ASAI-II级,随机均分为两组,硬膜外腔注入3%盐酸氯普鲁卡因(A组),注入2%碳酸利多卡因(B组),监测并记录两组患者麻醉前(T1),注药后5min(T2),15min(T3),25min(T4),55min(T5)各时点的SBP、DBP、HR、RR、SPO2,记录两组产妇镇痛起效时间,运动阻滞起效时间,镇痛持续时间,运动阻滞持续时间,采用针刺法测定痛觉消失最高平面,采用改良Bromage分级评价下肢运动神经阻滞最大程度:0级为无运动阻滞,1级为不能抬起大腿,2级为不能屈膝,3级为不能屈踝。结果:A组镇痛起效时间、运动阻滞起效时间、镇痛持续时间及运动阻滞持续时间短于B组(P<0.01),A组产妇运动阻滞最大程度小于B组(P<0.01),痛觉消失最高平面比较差异无统计学意义,两组麻醉前后不同时点SBP、DBP、HR、RR比较差异无统计学意义。A组与B组比较,T2、T3、T4、T5的SPO2均高于B组。结论:3%盐酸氯普鲁卡因用于剖宫产手术起效快;恢复快;镇痛肌松完善;毒副作用低;适用于产科麻醉。Objective： To compare the same dose of 3% chlorine procaine hydrochlofide, 2% carbonated lidocaine for epidural anesthesia for cesarean section surgery anesthesia effect. Methods： 180 cases of cesarean section surgely patients, ASAI - class II, were divided into two groups at random, the epidural cavity injection of procaine chloride （group A） and injection of lidocaine calbonate （group B）. Monitor and record before anesthesia（T1 ）, two groups of patients with drug ＇after 5min （T2）, 15min （T3）, 25min （T4）, 55min （T5）various point of SBP, DBP, HR, RR, SPO2. Records of two groups of maternal analgesia effect time, motion block effect time, duration of analgesia, motion block duration, pain disappear highest level was evaluated by acupuncture method, modified Bromage grading evaluation of lower limb motor nerve block： maximum level 0 for no motor block, level 1 for couldn＇t lift the thigh, level 2 for not bend your knees, level 3 for ankle flexion. Results： Group A analgesic effect time, anesthesia effect time and motion block motion duration shorter than group B （P〈0.05）, two groups of maternal pain disappear the highest level, motion block maximum duration, analgesia, there was no statistically significant difference comparing two groups of difterent time points before and after anesthesia SBP, DBP, HR, RR, SPO2 comparison difference has no statistical significance. Conclusion：3% procaine chloride used in cesarean section surgely effect fast, anesthesia effect is exact, low toxic and side effect, rarely through the placenta, suitable for obstetric anesthesia.
