摘　　要：随着综合卫生改革的不断深入,上海市松江区社区卫生服务的一些问题日益突出。通过转变社区卫生医务人员的理念,开展家庭医生制服务,实现信息化双向转诊和利用信息平台进行考核,缓解了卫生资源短缺、人才不足和绩效考核困难的问题。With the deepening development of comprehensive health reform, there are some problems of community health services existing in Songjiang District of Shanghai. By transforming thoughts of community health medical staffs, family doctor service system is established, atwo-way referral and the assessment based on information platform are implemented to alleviate the difficulties of the shortage of health resources, lacking trained personnel and difficult to evaluate performance.
