白细胞介素17/23与重症急性胰腺炎的关系研究及意义

摘　　要：目的通过建立大鼠重症急性胰腺炎(Severe acute pancreatitis,SAP)模型,观察白介素-17(IL-17)、白介素-23(IL-23)与重症胰腺炎的关系及地塞米松对IL-17表达的影响,探讨IL-17/23在SAP发病机制中的作用。方法 SD大鼠,共45只,随机分为三组(n=15)。对照组(A组):正常组;模型组(B组):重症急性胰腺炎组;干预组(C组):重症急性胰腺炎+地塞米松组。每组按3h、6h、12h又随机分成三小组(n=5)。重症胰腺炎的制作通过于胰胆管逆行注射5%的牛黄胆酸钠造成。三组大鼠分别于造模后3h、6h、12h经下腔静脉取血测定血淀粉酶的水平,同时,严格按照酶联免疫实验的要求,检测血清IL-17、IL-23的水平。观察胰腺标本大体改变并取胰腺组织HE染色,做胰腺病理学评分。结果 B组血清IL-17、IL-23水平明显高于A、C二组(P<0.01),且随时间变化,差异越显著;C组血清IL-17、IL-23水平明显高于A组(P<0.01),而随时间变化,C组各时间点相比差异无统计学意义;胰腺大体观及病理评分:B组胰腺病理评分结果明显高于A、C二组(P<0.01),且随时间变化,差异越显著;C组病理评分结果明显高于A组(P<0.01),而随时间变化,C组各时间点相比差异无统计学意义。结论 IL-17/IL-23表达水平与胰腺炎的严重程度相关,地塞米松可抑制IL-17表达,改善SAP的严重程度。Objective To observe the dynamic change of interleu - kin- 6（IL- 17）, interleu - kin- 23 （IL- 23） in rats with severe acute pancreatitis（SAP） to approach the effection of IL - 17/23 in SAP pathogenesy. Methods 45 healthy rats were randomly divided into three groups： control group（group A） ; model group（group B） ; Severe acute pancreatitis intervention group （group C） ： dexamethasone ＋ Severe acute pancreatitis group） ;any of the three groups were randomly divided into three groups （3h,6h,12h） ; SAP modle was induced by injection of 5% sterile sodium taurocholate into biliopancre- atic duct. Rats . Rats Blood sample were collected through inferior caval vein after operated to examine IL- 17, IL- 23 with enzyme linked immunosorbent assay（ELISA） ;We used AEROSET automatic biochemical analyzer to inspection triglyceride, the amount and the color of ascites; Schmidt scoring method was used to evaluate pancreatic pathology. Results 1. group B had higher IL- 17 /IL- 23 expression than A and group C （P〈0.01）, and in three periods had significant changes （P〈0.01）; group C had higher IL- 17/IL- 23 expression than group A in the serum（P〈0.01） ;but group C was no significant changes in three periods; 2. group B had higher pancreatic pathology scored than group A and group C（P〈0. 01） ,and in three periods had significant changes（P〈0. 01） ; group C had higher pancreatic pathology scored than A group（P〈0.01） ;but group C was no significant changes in three periods. Conclusion The expression of IL - 17/IL - 23 is closely related to the severity of AP, Dexametha sone could derease the expression of IL- 17/IL- 23, thus leading to intiibiting the progression of AP.
