摘 要:目的探讨肝硬化不同时期各级肾动脉血流参数特点,为肝硬化临床分期、诊治及预后评估提供依据。方法选取我院2010年7月至2012年9月确诊的不同程度肝硬化患者103例作为肝硬化组,根据病情将其分为肝硬化代偿期组(Ⅰ组)37例、肝硬化失代偿期组(Ⅱ组)38例、肝肾综合征组(Ⅲ组)28例。选取同期49例健康体检者为对照组。应用彩色多普勒超声监测各组肾主动脉(MRA)、段动脉(SRA)、叶间动脉(IRA)的血流参数。结果随着肝硬化病情的加重,MRA、SRA、IRA的阻力指数(RI)、搏动指数(PI)逐渐增加,收缩期最大血流速度(Vmax)、舒张末期最低血流速度(Vmin)及肾血流量(Q)逐渐变小。Ⅰ组各级肾动脉血流参数与正常组比较差异均无统计学意义(P>0.05);Ⅱ组、Ⅲ组RI、PI均显著高于正常组(P<0.05);Ⅱ组、Ⅲ组Vmax、Vmin及Q显著低于正常组(P<0.05)。结论肝硬化疾病发展进程中各级肾动脉的血流量逐渐下降,而RI和PI逐渐升高。监测各级肾动脉的血流量的变化对肝硬化疾病的诊治及预后有重要价值。Objective To investigate the parameters' characteristics of blood flow at all levels of renal artery in the process of cirrhosis and provide evidence for clinical staging,diagnosis,treatment and prognosis of cirrhosis.Methods One hundred and three patients with cirrhosis with varying degrees of cirrhosis from July 2010 to September 2012 in our hospital were collected as the liver cirrhosis group,which were divided into compensated cirrhosis subgroup (group Ⅰ),decompensated cirrhosis subgroup (group Ⅱ),hepatorenal syndrome subgroup (group Ⅲ) according to the course of disease.Forty-nine healthy individuals were collected as the control group at the same time.Color Doppler ultrasound was used to detect blood flow parameters of the main renal artery (MRA),segmental artery (SRA),interlobar artery (IRA).Results As illness progresses,resistance index (RI) of MRA,SRA,IRA,pulsatility index (PI) gradually increased,peak velocity in systolic (Ⅴmax),minimum blood flow velocity in diastolic (Ⅴmin) and renal blood flow (Q) decreased gradually.Blood flow parameters of all levels of renal artery between group Ⅰ and the control group showed no statistically significant difference (P>0.05).RI,PI of group Ⅱ,group Ⅲ were significantly higher than the control group (P<0.05).Ⅴmax,Ⅴmin and Q of group Ⅱ,group Ⅲ were significantly lower than the control group (P<0.05).Conclusion In the development of cirrhosis,blood flow of renal artery at all levels decreases gradually,while RI and PI increase gradually.Monitoring changes of blood flow of renal artery are important to the diagnosis and prognosis of cirrhosis.
