丙种免疫球蛋白联合低分子肝素治疗不明原因复发性流产的临床观察

摘　　要：目的探讨静脉注射丙种免疫球蛋白联合皮下注射低分子肝素治疗不明原因复发性流产的效果。方法选取本院2006～2013年收治的不明原因复发性流产早期妊娠患者260例，随机分为观察组160例和对照组100例。对照组采用黄体酮注射液及HCG治疗，观察组在对照组基础上采用丙种免疫球蛋白联合低分子肝素治疗，观察两组的疗效及不良反应发生率。结果观察组继续妊娠至12周成功率为85．0％。对照组继续妊娠至12周成功率为50．O％，差异有统计学意义（P〈0．05）。观察组再次自然流产率为13．1％。对照组再次自然流产率为48．0％。差异有统计学意义（P〈0．05）。两组异位妊娠发生率及不良反应发生率比较差异无统计学意义（P〉0．05）。结论采用传统安胎药黄体酮注射液、HCG，联合丙种免疫球蛋白、低分子肝素治疗不明原因复发性流产患者，可提高安胎成功率，药物不良反应发生率低。Objective To study effect on intravenous injection of gamma immunoglobulin combined with subcutaneous injection of low molecular weight heparin in the treatment of unexplained recurrent spontaneous abortion.Methods 160 cases with unexplained recurrent spontaneous abortion patients in our hospital from 2006 to 2013 were selected and randomly divided into observation group（160 cases） and control group（100 cases）.Progesterone injection and HCG were used in control group,on the basis of control group,gamma immunoglobulin combined with low molecular weight heparin were used in observation group.Effect and incidence rate of adverse reaction in two groups were observed.Results The pregnancy continued to 12 weeks success rate of observation group and control group was 85.0% and 50.0% respec- tively,with statistical difference（P〈0.05）.Once again the natural abortion rate of observation group and control group was 13.1% and 48.0% respectively,with statistical difference（P〈0.05）.The incidence rate of ectopic pregnancy and incidence rate of adverse reaction in two groups was compared respectively,with no statistical difference（P〉O.05）.Conclusion Pro- gesterone injection,HCG,gamma immunoglobulin and low molecular weight heparin in the treatment of unexplained re- current spontaneous abortion patients can improve success rate of pregnancy,incidence rate of adverse reaction is low.
