经皮微创螺钉内固定治疗骨盆骨折局限性探究

摘　　要：目的探究经皮微创螺钉内固定手术治疗骨盆骨折的临床疗效,评价其适用性与局限性。方法回顾性分析我院2008—2012年25例经皮微创螺钉内固定手术治疗骨盆骨折的临床资料。结果本组25例,单纯前环骨折15例,前后环骨折10例,单侧耻骨坐骨骨折2例,双侧耻骨坐骨骨折伴耻骨联合分离11例,骶髂关节脱位4例,骶骨骨折1例。均经皮微创螺钉内固定手术治疗后恢复良好。Majeed功能评分,优15例,良4例,中、差6例,优良率为76%。结论经皮微创螺钉内固定手术治疗骨盆骨折效果良好,符合手术向微创方向发展的趋势,但由于骨盆区的特殊结构及高度变异率、术中影像学的局限性、操作风险高等,实施存在一定的局限性。Objective To explore the curative effect of minimally invasive percutaneous screw fixation in treatment of pelvic fractures.and to evaluate its applicability and limitations.Methods Clinical data of 25 patients undergoing minimally invasive percutaneous screw fixation in treatment of pelvic fractures,from 2008 to 2012 was retrospectively analyzed.Results Of the 25 patients,there were 15 patients with simple anterior pelvic ring fractures,10 patients with posterior and anterior pelvic ring fractures,2 patients with unilateral pubic sciatic fractures,11 patients with bilateral pubic sciatic fractures associated with puhic symphysis separation,4 patients with sacroiliac dislocations and 1 patient with sacral fractures.All the patients recovered well after minimally invasive percutaneous screw fixation.There were 15 patients being excellent,4 patients being well and 6 patients heing poor by the Majeed Function Scoring System,and the fineness rate was 76％.Conclusion Minimally invasive percutaneous screw fixation in treatment of pelvic fracture is effective,but it has a certain degree of limitations for performance due to the special characteristics and high aberration rate of the pelvis area,intraoperative iconography limitation and high operative risks.
