益肺活血涤痰汤治疗慢性肺原性心脏病缓解期患者45例临床观察

摘　　要：目的观察益肺活血涤痰汤治疗慢性肺原性心脏病(简称慢性肺心病)缓解期患者的临床疗效。方法将90例慢性肺心病缓解期患者随机分为治疗组和对照组各45例,对照组予吸氧、抗感染、止咳平喘、强心等西医常规治疗,治疗组在上述治疗基础上加服益肺活血涤痰汤,每日1剂,2周为1个疗程,共治疗2个疗程。比较两组临床疗效及治疗前后血气分析、血液流变学和肺功能指标的变化。结果治疗组临床疗效总有效率为91.1%,对照组为75.6%,两组比较差异有统计学意义(P<0.01)。与治疗前相比,两组患者治疗后动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)均改善(P<0.05),且治疗组改善明显优于对照组(P<0.05)。与治疗前相比,治疗组患者治疗后全血高切黏度、全血低切黏度、血浆黏度、纤维蛋白原、血小板凝聚率下降(P<0.05),且治疗后治疗组患者血液流变学各项指标均优于对照组(P<0.05)。与治疗前相比,两组患者治疗后第一秒用力呼气量(FEV1)、最大通气量(MVV)和最大呼气流速(PEFR)都有显著改善(P<0.05),且治疗组患者肺功能改善明显优于对照组(P<0.05)。结论益肺活血涤痰汤能有效降低血浆黏度,抑制血小板聚集,改善微循环障碍和肺功能,提高临床疗效。Objective To observe the therapeutic effect of lung-boosting, blood-quickening and phlegm-flushing decoction （LBPD） for chronic pulmonary heart disease. Methods Ninety patients with chronic pulmonary heart disease in remission were randomized into the treatment group and control group, with 45 in each. The control group was given routine western medicine. The treatment group was added with LBPD one dose daily. The treatment lasted for 4 weeks （2 treatment courses）. The efficacy and the changes of blood gas analysis, hemorheology and lung func- tion were observed and compared. Results The total effective rate of the treatment group and control group was 91.1% and 75.6% respectively, with a significant difference between groups （P 〈0.01 ）. The values of PaO2 and PaCO2 were significantly improved in both groups after treatment （P 〈 0. 05 ）, especially in the treatment group （P 〈 O. 05）. The whole blood viscosity at high and low shear rates, plasma viscosity, fibrinogen and platelet aggregation rate were significantly decreased in both groups after treatment （ P 〈 O. 05 ）, especially in the treatment group （ P 〈 0. 05）. The forced expiratory volume in one second, maximum voluntary ventilation and peak expiratory flow rate were significantly improved in both groups after treatment （ P 〈 0.05 ）, especially in the treatment group （ P 〈 0. 05 ）. Conclusion LBPD can effectively reduce plasma viscosity, inhibit platelet aggregation and improve microcirculation, lung function and efficacy.
