摘 要:目的 探讨 1~ 12月龄健康婴儿肺功能的发育情况。方法 将 140名 1~ 12个月的健康婴儿分成 4组 :1~ 2月、~ 4月、~ 7月、~ 12月组。用美国森迪公司 2 6 0 0型儿童肺功能仪测定肺功能值 ,包括用潮气流速容量曲线部分替代最大呼气流速容量曲线检测出的潮气量和反映大小气道功能的指标 ;利用被动流速容量技术测定呼吸系统静态顺应性、总气道阻力 ;用开放式氮冲洗的方法测定功能残气量。结果 本组婴儿各项肺功能测定值男女间差异无显著意义 (P >0 .0 5 ) ,通气功能、功能残气量、呼吸系统的静态顺应性随月龄增大而增大 ,与身高、月龄、体重密切相关 ,各组间差异有显著性意义 (P <0 .0 1) ,而反映大、小气道功能所有指标都相对稳定。结论 用潮气流速容量曲线部分代替最大呼气流速曲线可反映大小气道功能情况 ,其各项指标相对稳定 ,可作为观察呼吸系统疾患时肺功能变化情况的比较理想的指标。通气功能、功能残气量、呼吸系统静态顺应性、潮气呼气峰流速随月龄增大而增大 ,符合婴儿生长发育的规律 ,1~ 4月婴儿发育最快。Objective To explore the development of pulmonary function of normal infants between 1-12 months. Methods Totally 140 normal infants were divided into 4 groups according to their age, i.e., 1-2 months, ~4 months, ~7 months, and ~12 months. Pediatric pulmonary function laboratory type 2600 (Sensor Medics Corporation USA) was used to detect tidal flow volume curve, which can partially replace the maximum expiratory flow volume curve. Parameters showing small airway function were ratio of peak tidal expiratory flow time to expiatory time, ratio of expiratory flow at 75% of tidal volume to peak tidal expiratory flow, and ratio of inspiratory time to total respiratory time. Parameters showing large airway function was ratio of mid-tidal expiratory flow to mid-tidal inspiratory flow. Passive expiratory flow volume technique was used to examine respiratory system static compliance and total airway resistance. Open nitrogen washout method was used to measure functional residual volume. Results No significant difference was found between male and female infants in each group ( P > 0.05), but values of tidal volume, peak tidal expiratory flow, respiratory system static compliance, residual volume were significantly different ( P <0.01) between every two groups. The values of each parameter mentioned above increased with age, body weight and height, and were positively correlated ( r =0.90,0.58,0.66,0.98). Respiratory rate decreased with the age. The tidal flow volume curve was untypical oval. Conclusion Tidal flow volume curve could show functions of small and large airway and partially replaces the maximum expiratory flow curve. The values of each parameter were stable in infants and could be used as reasonable index to observe the pulmonary function change in pulmonary diseases. The ventilatory function, residual volume, respiratory system static compliance and peak tidal flow increased with the age, especially during the first 1~4 months of life, which coincides with the rule of development of infa
