摘 要:目的:评价调补肺肾(补肺健脾、补肺益肾、益气滋肾)三法对慢性阻塞性肺疾病(COPD)大鼠肺组织基质金属蛋白酶(MMP)-2、MMP-9和组织金属蛋白酶抑制物(TIMP)-1 mRNA表达的影响及远后效应。方法:大鼠随机分为对照组、模型组、补肺健脾组、补肺益肾组、益气滋肾组和氨茶碱组,采用香烟熏吸联合细菌感染法制作COPD模型,于第9周起分别给予补肺健脾方、补肺益肾方、益气滋肾方及氨茶碱至第20周。于第20、32周观察大鼠肺组织病理变化和MMP-2、MMP-9和TIMP-1 mRNA的表达。结果:模型组大鼠肺组织病理损伤明显。第20、32周,模型组MMP-2、MMP-9和TIMP-1 mRNA较对照组明显升高(P<0.01);与模型组比较,3个中药组和氨茶碱组MMP-9、TIMP-1 mRNA明显降低(P<0.05,P<0.01),补肺健脾组和补肺益肾组MMP-2 mRNA明显降低(P<0.05,P<0.01);与氨茶碱组比较,第20、32周补肺健脾组MMP-2、MMP-9、TIMP-1 mRNA明显降低(P<0.05,P<0.01),第20周补肺益肾组和益气滋肾组TIMP-1 mRNA、第32周补肺益肾组MMP-2显著降低(P<0.05,P<0.01)。3个中药组间上述指标均以补肺健脾组和补肺益肾组降低尤为明显。32周时除对照组外各组上述指标较本组20周明显降低(P<0.01)。结论:调补肺肾三法可减轻COPD病理损伤,其良好的疗效及远后效应机制可能与降低COPD肺组织MMP-2、9和TIMP-1 mRNA的表达有关。Objective: To explore the influence and long-term effect of the three Tiaobu Feishen therapies, including invigorating lung and spleen (Bufei Jianpi), supplementing lung and kidney (Bufei Yishen) and nourishing qi and kidney (Yiqi Zishen), on matrix metalloproteinases (MMP)-2, 9 and tissue inhibitor of metaUoproteinase (TIMP)-1 mRNAs expressions in lung tissues in rats with chronic obstructive pulmonary disease (COPD). Methods: Rats randomized into control, model, Bufei Jianpi, Bufei Yishen, Yiqi Zishen and aminophyline groups were exposed to cigarette smoke and bacterial infections to product COPD rat model, and treated with Bufei Jianpi, Bufei Yishen, Yiqi Zishen Granules and aminophyline respectively from week 9 to 20, and them sacrificed at week 20 and 32. The pathological impairments and expressions of MMP-2, MMP-9 and TIMP-1 mRNAs in the lungs were determined. Results: Rats in the model group showed obvious pathological impairments in lung tissues. At week 20 and 32, the levels of MMP-2, MMP-9 and TIMP-1 mRNAs were higher than that in control rats, while MMP-9 and TIMP-1 mRNAs in the three Chinese medicine- and aminophylline-treated groups, MMP-2 mRNA in Bufei Jianpi and Bufei Yishen groups were lower than in model group (P〈0.05, P〈0.01). Compared with aminophylline-treated rats, MMP-2, MMP-9 and TIMP- 1 mRNAs were lower in Bufei Jianpi group at both week 20 and 32 (P〈0.05, P〈0.01), while TIMP-1 mRNAs were lower in Bufei Yisben and Yiqi Zishen groups at week 20 and MMP-2 mRNA in Bufei Yishen group at week 32 (P〈0.05, P〈0.01). There were more benefits in Bufei Jianpi and Bufei Yishen groups in reducing the levels of the above-mentioned parameters. There were also significant reducing even at week 32 compared with week 20 (P〈0.01). Conclusion: The three Tiaobu Feishen therapies can significantly reduce lung impairments in rats with COPD and have good long-term effects, and reducing the levels of MMP-2, MMP-9 and TIMP-1 might be involved in the acti
