摘 要:目的制备带有微孔循环的个性化羟基磷灰石(Hroxyapatite,HA)人工甲状软骨支架及其性能的测试与临床应用研究。方法采用一种新的甲状软骨支架结构设计和制造方法,以羟基磷灰石为主要材料,依据原位固化方式,通过添加有机单体和交联剂形成网格。并通过添加表面活性剂TrifionX—114,降低浆料表面张力,稳定气泡,采用机械搅拌起泡法从而获得一定孔隙大小的泡沫浆料。结合凝胶注模法,脱模获得个性化甲状软骨陶瓷支架,测试其性能,然后采用冷冻吸附法使水溶性I型胶原与重组人骨形态发生蛋白-2混合溶液沉淀于陶瓷支架上,制备个性化HA人工软骨复合支架,最后进行动物甲状软骨损伤修复的应用测试实验。结果HA泡沫陶瓷孔隙尺寸集中于150~300μm范围,孔隙率为(74.37±6.97)%,生物力学强度为(2.3±0.2)mPa。组织学观察,植入HA人工软骨复合支架动物组显微镜下可见明显的新生骨生长及表现出良好的发音、饮食行为。结论验证了个性化HA人工软骨复合支架制备与临床应用的可行性。Objective Preparation of personalized artificial hydroxyapatite thyroid cartilage framework with circulato- ry micropore and its performance test, then study on clinical practice. Methods In this paper, we present a new method of thyroid cartilage framework design and manufacture. According to the Cured - In - Place - Pipe, with hydroxyapatite as main starting material, we add the monomer and cross linker, then the grid structure forms. In order to reduce the slurry's surface tension and stable the bubbles, we add the surfactant TritionX - 114, then use the mechanical stirring Direct Foaming method to attain a certain pore size of open pore ceramics slurry. Basing on this, combined with gel - casting process, Demould to ob- tain personalized ceramic thyroid cartilage bracket. Test its properties afterwards. Next, freezing - adsorption method is used to make water- soluble collagen and recombinant human bone morphogenic protein- 2 on the ceramic support. Finally, we got the personalized artificial composite thyroid cartilage framework and did the experiment on animals for repairing thyroid cartilage defects. Results The pore size of HA foam ceramic focus on 150 μm to 300 μm. Its porosity ratio is ( 74. 37 ±6. 97 )%. And the biomechanical strength is (2. 3 ± 0. 2) MPa. Histological observations on the animals that transplanted ar- tificial composite cartilaginous framework showed that, new bone was found in the defect. Besides, the representation of ani- mals' phonation and diet behaved well. Conclusion By the clinical application of thyroid cartilage damage repairing on the animal, the feasibility of utilization of this research project can be verified.
