摘  要:目的探讨多层螺旋CT容积漫游(VRT)及重组技术对无名动脉压迫综合征的诊断价值。方法搜集郑州市儿童医院2010年12月至2013年5月间临床有喘息、喉鸣、呼吸困难、反复呼吸道感染,高度怀疑气道受压患儿7例,行CT及纤维支气管镜检查,扫描数据在工作站进行多平面重组、VRT、VR透明化重组,分析其影像学特点。结果 7例中4例为无名动脉起源正常,无名动脉发出后走形异常,上升支偏左,压迫气管,2例为无名动脉起始点偏后,压迫气管,1例为右位主动脉弓,无名动脉位于左侧;其中4例伴发先天性心脏病,1例为动脉导管未闭,2例为室间隔缺损,1例法洛四联症。结论 VRT及VR透明化重组可立体显示畸形血管的构造及走行,气管、支气管狭窄部位、范围、程度,具有良好对比度及锐利度,且解剖结构更易于辨认,用于无名动脉压迫综合征的诊断是最佳选择。Objective To explore the diagnostic value of the innominate artery pressure syndrome with multi-slice spiral CT VRT and the reconstruction technique. Methods 7 cases of children with tracheal compression confirmed through op- eration from Dec. 2010 to Dec. 2012 were collected in Zhengzhou Children~ Hospital in this study. All cases had underg- one large blood vessels'thin slice CT contrast enhancement. The data were transmitted to the work station for multi planar reconstruction (MPR), volume rendering technique (VRT) and VR transparency reconstruction for the analysis of the ima- ging characteristics. Results In the 7 cases, 5 were normal in origin of innominate artery, and became abnormal when the innominate artery, with the ascending branch deviated to left, compressing the trachea, 2 cases'innominate artery devi- ated to the right, compressing the trachea, 3 cases were accompanied with congenital heart disease, 1 was patent ductus ar- teriosus, 2 cases with ventricular septal defect. Conclusion Volume rendering technique (VRT) and VR transparency reconstructiion could display three-dimensionally the structure and abnormality of the deformative blood vessel, the part, scope and the degree of the congenital trachea bronchiostenosis. It could display good trachea, contrast and sharpness, making anatomical structure easy to identify. It is the best choice for the diagnosis of the innominate artery compression syn- drome.
