摘 要:目的观察肾移植术后随访患者血清中SOD的活性和β2微球蛋白含量的变化,研究SOD和β2微球蛋白在检测慢性移植肾肾病中的诊断价值。方法将肾移植术后随访的患者和健康查体的人群分成慢性移植肾肾病组、移植后肾功能稳定组、移植后肾功能异常组(经移植肾穿刺活检排除是慢性移植肾肾病的其他类型肾病患者)、健康对照组,检测各组研究对象血清中SOD的活性、Cr和β2微球蛋白的浓度,并比较各组间的差异。结果与健康对照组比较,慢性移植肾肾病组、移植后肾功能异常组和移植后肾功能稳定组患者血清中SOD活性明显降低,差异有统计学意义(P<0.01);其中慢性移植肾肾病组显著低于移植后肾功能异常组和移植后肾功能稳定组,差异有统计学意义(P<0.01);肾功能异常组和肾功能稳定组差异无统计学意义(P=0.442)。慢性移植肾肾病组患者血清中β2微球蛋白的浓度明显高于移植后肾功能异常组、移植后肾功能稳定组和健康对照组,差异有统计学意义(P<0.01);移植后肾功能异常组亦明显高于移植后肾功能稳定组和健康对照组,差异有统计学意义(P<0.01);移植后肾功能稳定组和健康对照组差异无统计学意义(P=0.846)。结论检测SOD活性和β2微球蛋白含量对鉴别慢性移植肾肾病有一定的参考价值。Objective To evaluate the clinical values of serum SOD activity and the levels of 132-microglobulin in the detection and diagnosis of chronic allograft nephropathy. Methods Kidney transplant patients and healthy subjects were divided into four groups: chronic allograft nephropathy group, stable renalfunetion group (after renal allograft), abnormal renal function group (after renal allograft biopsies from chronic allograft nephropathy and other types of nephrotic patients), and the normal group. The activity of SOD, level of 132-microglobulin and Cr concentrations were measured. Results Compared with the normal group, the activity of SOD in the other three groups was significantly reduced (P〈0.01). Activity of SOD in the chronic allograft nephropathy group was lower than the other two transplant groups (P〈0.01). No significant differencewas observed between the abnormal renal function group and the stablerenal function group (P= 0.442). The concentration of serum 132-microglobulin in the chronic allograft nephropathy group was higher than the other groups (P〈0.01). The concentration of serum 152-microglobulin in the abnormal renal function group was higher than the stablerenal function group and the normal group (P〈0.01). Significant difference between the stable renal function group and normal group was not observed (P=0.846). Conclusion The activity of SOD and the concentration of 132-microglobulin may be used in the diagnosis of chronic allograft nephropathy.
