2008-2011年遵义市医疗机构传染病网络直报质量分析

摘　　要：目的了解遵义市各级医疗机构传染病漏报及网络报告质量情况,为今后降低医疗机构传染病漏报及规范网络直报提供科学依据。方法 2008-2011年均按照全国及贵州省相关要求结合遵义市实际情况制定检查方案,运用Excel表格对各年传染病漏报调查原始资料进行统计分析。结果遵义市2008-2011年医疗机构平均漏报率1.51%,其中门诊漏报率1.72%,住院部漏报率0.94%;按病种统计乙类传染病漏报率为2.09%,丙类传染病漏报率为1.10%;按医疗机构级别统计,地市级及省级医疗机构漏报率0.91%,县级医疗机构漏报率1.26%,乡镇卫生院漏报率2.19%;传染病报告卡质量:纸卡完整率及准确率分别为96.00%、95.63%,纸卡与日志/出入院登记一致率82.73%,纸卡与网卡一致率91.42%,网卡报告及时率98.05%;门诊日志登记符合率:2008-2011分别为71.32%(265/189)、73.95%(572/423)、74.89%(657/492)、77.28%(625/483)。结论遵义市医疗机构门诊日志登记符合率较低,存在传染病漏报隐患、纸卡与日志/出入院登记一致率及纸卡与网卡一致率均较低,下一步还需加强医疗机构传染病报告及网络直报质量规范化管理。Objective To investigate the underreporting of infectious diseases and quality of network reporting of medical institutions at all levels, to provide scientific basis for reducing omissions of reports and standardly managing direct network reporting in the fu- ture. Methods In accordance with the relevant requirements in China and Guizhou Province from 2008 to 2011 and combined with detailed situations in Zunyi, formulated check program, used Excel for statistical analysis on raw data of the infectious diseases. Re- suits The average missing report rate was 1.51% in medical institutions in Zunyi from 2008 to 2011, in which 1.72% among outpa- tients, 0.94% in inpatients; missing report rate of category B notifiable disease was 2.09% category C notifiable disease was 1.10%; by medical institutions, missing report rate of municipal and provincial level institutions was 0.91%, at the county level was 1.26%, 2.19% in township hospitals; quality of infectious disease report cards： completion rate and accuracy rate of paper cards were 96.00%, 95.63%, concordance rate of paper card and log, discharge the admission was 82.73%, concordance rate of paper card and network card was 91.42%, timely rate of network report was 98.05%; outpatient and log registration concordance rate： from 2008 to 2011, the rate was 71.32% （265/189）, 73.95% （572 / 423）, 74.89% （657 / 492）, 77.28% （625 / 483）, respectively. Conclusion The concordance rate of outpatient and log registration is low, there is some missing report, concordance rates of paper card and log, dis- charge the admission, and of paper card and network card are low It should strengthen standardized management on quality of infectious disease report and direct network report.
