摘 要:目的:为了加强内镜清洗消毒的规范化管理,避免由内镜检查引起的交叉感染,本文根据排队论提出一种监督内镜清洗消毒操作规范执行情况的方法模型。方法:分析内镜洗消、内镜使用过程中时间上的连续性,分析内镜、床位和洗消槽使用过程中三者之间的同步性,当三者之间的关系比较简单时,推导出计量单日内镜最大诊疗量的数学公式;当三者之间的关系比较复杂时,用"生产者-消费者"设计模式来模拟仿真内镜消洗的排队过程,据此设计软件模型用以计算单日内镜最大诊疗量,并依据计算结果来分析内镜消毒流程的时间是否得以保证,判断内镜清洗消毒操作是否符合规范要求。结果:给定内镜室有l条内镜、m张床位和n套洗消槽,当l<m且l=n或l>m且l≤n时,可由推导出的数学公式计算出单日最大诊疗量及诊疗人序;当l>m且l>n或l、m、n是任意组合时,利用模型软件设定好l、m和n的数值,即可计算出单日最大诊疗量及诊疗人序。结论:通过推导数学公式和设计模型软件两种方法来计算单日内镜最大诊疗量,可方便常态性的检查、自查清洗消毒流程的时间是否得以保证,还可以通过该计算软件给出床位、内镜和洗消槽的合理配置。Objective: In order to strengthen the standardized management of endoscopic decontamination and disinfection to prevent cross-infection, we propose a method of supervision of endoscopic decontamination and disinfection operations in accordance with the implementation of the model specification queuing theory. Methods: We analyze continuity course of time when cleaning and using endoscopic, and analyze the synchronized relationship of endoscopy, beds and decontamination tank. We get a mathematical formula to measure the largest amount of endoscopic treatment in a single-day, when their relationship is relatively simple; we design software model with "producer - consumer" design pattern to simulate the queuing process of cleaning endoscope, when their relationship is more complicated. By using of it to calculate the amount of endoscopic treatment in a single-day, and based on the results to analyze whether the time is guaranteed to endoscopic cleaning and disinfection operations with regulatory requirements. Results: Given there are 1 endoscopy rooms, m beds and n sets of decontamination tank, when (l〈m and l=n ) or ( 1 〉 m and l=n), a mathematical formula can calculate the largest amount of endoscopic treatment in a single-day; when ( l 〉 m and I 〉 n) or l-m-n is random, the computing software can calculate the largest amount of endoscopic treatment in a single-day. Conclusions: By the computing software of calculating the maximum amount of endoscopic treatment, it is useful to facilitate the inspection of normality, and to guarantee the time of self-examination, cleaning and disinfection processes. By this software we can also get the reasonable arrangement of beds, endoscopies and decontamination tanks.
