经胸及经多平面食管超声诊断先天性主动脉四叶瓣畸形

摘　　要：目的 :探讨经胸 (TTE)和多平面经食管超声心动图 (MTEE)诊断主动脉四叶瓣 (QAV)的价值。方法 :采用 HP2 5 0 0、 5 5 0 0型彩色多普勒超声心动图诊断仪 ,TTE探头频率 2 .0～ 4.0 MHz;MTEE探头频率 5 .0～6 .2 MHz。重点观察主动脉瓣的数目、形态、回声、附着点、关闭线、启闭 ,冠状动脉起源和血流动力学。结果 :TTE确诊 QAV 5例 ,MTEE确诊 3例 ,其中 3例经手术证实。男 4例 ,女 4例 ,年龄 35～ 5 5岁 ,平均 41.1岁。在 8例QAV中 ,四个叶大致相等 5例 (A型 ) ,其余 3例四叶大小不对称 ,一个瓣叶较小 ,另外三个瓣叶大致相等 ,呈三大一小型 (B型 )。多普勒显示 A型中 4例轻度和 1例中度主动脉瓣返流 ,B型中 2例中度和 1例重度主动脉瓣返流。结论 :经胸和多平面经食管超声能诊断先天主动脉四叶瓣畸形 。Objective:To evaluate the value of TTE and MTEE in diagnosing quadricuspid aortic valve(QAV).Methods:TTE and MTEE were performed in 8 cases.The number,conformation,echo,opening and closing of the aortic valve,origin of coronary arteries as well as the hemodynamics were analyzed by HP 2500 and HP 5500 imaging systems.Transducer frequency 2 0～4 0 MHz for TTE and 5 0～6 2MHz for MTEE were used.Results:The ages of the 8 cases ranged from 35 to 55 years(mean 41 1).QAV patients had an equal sex distribution.Five cases were diagnosed as QAV by TTE and 3 by MTEE.Of the 8 QAV patients reviewed,5 cases had 4 equal cusps(type A),and the other 3 cases had 3 equal sized cusps and 1 smaller cusp(type B).In type A,Doppler examination showed mild aortic regurgitation(AR)in 4 and moderate AR in 1.In type B,Doppler examination showed moderate AR in 2 and severe AR in 1.Conclusion:Diagnosis of QAV can be made by TTE and MTEE which is important for treatment.
