浙江瑞安市健康体检人群慢性肾脏病患病率及相关危险因素分析

摘　　要：目的调查浙江瑞安市健康体检人群慢性肾脏病(CKD)患病率及相关危险因素。方法选择年龄≥18周岁的体检人群资料43 690例,进行CKD流行病学调查及相关危险因素分析。结果在资料完整的43 690例成年体检者中,CKD患病率15.06%,其中男13.31%,女17.84%,差异有统计学意义(P<0.01);尿红细胞阳性率13.63%,其中男11.82%,女16.97%,差异有统计学意义(P<0.01);尿蛋白阳性率5.83%,男3.48%,女9.78%,差异有统计学意义(P<0.01);CKD各期患病率:CKD1期13.25%、CKD2期1.53%、CKD3期0.18%、CKD4期0.09%、CKD5期0;二分类logistic回归分析显示:性别(女性)、高收缩压、高尿酸血症、高胆固醇是CKD的独立危险因素。结论瑞安市健康体检人群的CKD患病率高于其他城市,在沿海经济发达的地区开展CKD的早期防治更为迫切。Objective To investigate the prevalence and associated risk factors of chronic kidney disease in adult popu- lation receiving healthy examination from urban area of Rui＇an, Zhejiang. Methods A total of 43 690 people （≥ 18 years） re- ceived healthy examination were included, and the epidemiology and risk factors of CKD be analyzed. Results The prevalence rate of C KD was 15.06% among the 43 690 cases, and the male/female prevalence ratio was 13,31%：17.84%（P〈0.01）. Among the people with CKD, the prevalence rate of hematuresis was 13.63%, and the male/female prevalence ratio was 11.82%： 16.79%,（X2=230.74, P〈0.01）, the prevalence rate of proteinuria was 13.63%,（the male/female prevalence ratio was 2.72%： 8.6%, X2=755.16, P〈0.01）. The prevalence rates of each state of CKD were 13.25%, 1.53%,0.18%,0.09% and 0%. Binary logistic regression analysis indicated gender （female）, high systolic blood pressure, high uric acid and high cholesterol were indepen- dently associated with CKD. Conclusion The prevalence rate of CKD in healthy crowd from urban area of Rui＇an is higher than other cities.In the coastal economically developed areas, the early preventions of CKD were more important.
