摘　　要：问卷是健康管理研究过程中用于采集受检者健康信息的重要工具,故问卷设计的合理性是影响调查结果客观性的关键因素。本中心从问卷的结构、内容及服务价值3个方面入手,以健康危险因素及健康评估作为内容设置的重点,以生理一心理一社会医学模式为指导,结合科学的数据管理及统计学方法论述问卷的合理设计,以理清问卷设计的思路及价值的体现。The questionnaire is the tool to acquire health information of the health management research , so the rationality of the questionnaire design will influence the results of the survey of research .From the aspects of questionnaire structure , content and service value , and setting the contents of the health risk factors and health as-sessment as the focus , guided by the physical -psychological -social medical model , the design of the question-naire and value were clarified with scientific data management and statistical theories .
