摘  要:目的:探讨抗菌药物临床合理使用管理模式。方法:建立医院抗菌药物使用管理小组,采用综合干预措施,随机抽取2010年(干预前)和2012年(干预后)出院患者病历各630份、门急诊处方各810张,进行对照研究。结果:干预前后比较,住院患者抗菌药物使用率由87.93%下降至52.06%,抗菌药物使用强度由87.5 DDD下降至39 DDD,微生物送检率由27.06%上升至86.03%,Ⅰ类切口手术预防用药比例由89.55%下降至28.03%,门诊处方抗菌药物比例由53%下降至15%,差异均具有统计学意义。结论:宣传培训、量化指标、行政干预、监控管理等综合干预,可提高临床合理使用抗菌药物的水平,对预防医院感染起到积极的促进作用,达到预期目标。OBJECTIVE:To explore the management on clinical rational use of antibiotics. METHODS: A team responsible for management on clinical rational use of antibiotics in hospital was set up and comprehensive intervention measures were taken. Each 630 medical records of discharged patients and 810 prescriptions in outpatient and emergency department before intervention ( in 2010 ) and after intervention ( in 2012 ) were randomly selected for a control study. RESULTS: The rate of antibiotic use reduced to 52.06% after intervention, as compared with 87.93% before intervention ; the antibiotic use density (AUD) reduced from 87.5 DDD to 39 DDD ; the rate of patients who received pathogen detection increased from 27.06% to 86.03% ; the rate of prophylactic drug use in type I incision patients reduced from 89.55% to 28.03% and the rate of outpatient antibiotic prescriptions decreased from 53% to 15%, all showing statistical differences. CONCLUSION: The publicity and training, quantification of indexes, administrative intervention, supervision and management resulted in improvement in the level of clinical rational use of antibiotics and achievement of the anticipated objective of hospital infection prevention.
