摘  要:目的观察胰岛素泵治疗围手术期糖尿病患者高血糖的疗效。方法 151例基本条件相似的糖尿病手术患者分为胰岛素泵组(CSⅡ)76例和多次皮下注射胰岛素组(MSⅡ)75例,比较其围手术期的血糖和胰岛素用量;将胰岛素泵组(CSⅡ)76例分为术后禁食组45例和非禁食组31例,比较其预期血糖控制标准达标率。结果术后3d CSⅡ组各时段血糖均较MSⅡ组下降显著(P<0.05),达到预期强化控制目标所需天数:CSⅡ组显著低于MSⅡ组(P<0.05),CSⅡ组较MSⅡ组所需胰岛素少(P<0.01),术后禁食组血糖达标率高于术后非禁食组(P<0.05)。结论糖尿病患者在围手术期CSⅡ比MSⅡ控制血糖更快、更好,是糖尿病患者围手术期高血糖治疗的首选。Objective To survey and compare the effects of continuous subcutaneous insulin injection(CSII) with multiple subcutaneous insulin injection(MSII) in per operative period. Methods 151 cases with diabetes mellitus during perioperative pe- riod were divided into the continuous subcutaneous insulin injection group( CSII group,76 cases)with multiple subcutaneous insu- lin injection group( MSII group,75 cases) ,The effects were compared between pre-operation and post-operation. Results In three days of post-insulin therapy for CSII group, the each time blood glucose (BG) was significantly lower than MSII group, P 〈 0. 05 ; the insulin dosage of CSII group was smaller than MSII group, P 〈 0. 05. Conclusion CSII was faster and better on regulating BG and fewer in hypoglycemia than MSII in per operative period. CSII was better on rehabilitation than MSII.
