慢性阻塞性肺病急性加重与凝血功能异常的相关性研究

摘　　要：目的 探讨纤维蛋白原、D-二聚体检测在慢性阻塞性肺病急性加重期的临床意义.方法 选择2011年1月～2012年10月在我院住院的AECOPD患者102例,COPD稳定期患者80例,检测纤维蛋白原、D-二聚体、PT、APTT、TT等凝血功能指标,同时作血气分析及肺功能检测,选择同期52例门诊健康体检者作凝血功能检测.结果 COPD急性加重期及稳定期患者血浆纤维蛋白原、D-二聚体明显高于正常对照组;急性加重期患者血浆纤维蛋白原、D-二聚体明显高于稳定期患者,差异均有统计学意义（P〈0.05）;随着COPD程度的加重,纤维蛋白原、D-二聚体水平也逐渐升高,差异有统计学意义（P〈0.05）.结论 慢性阻塞性肺病可引起血浆纤维蛋白原、D-二聚体的升高,导致凝血机制异常,体内处于高凝状态,其严重程度与病情成正相关.Objective To investigate the clinical significance of changes of plasma fibrinogen and D-dimer in patients with acute exacerbation of chronic obstructive pulmonary diseases （AECOPD）. Methods 102 COPD patients in acute exacerba- tion and 80 COPD patients in remission were included in the study from January 2011 to October 2012 in our hospital. 52 healthy persons were set as the control group. The plasma fibrinogen and D-dimer levels and active partial thromboplstin time （APTT）, prothrombin time （PT） and thrombin time （TY） were measured in all persons. Blood gas analysis and pulmonary function were detected in COPD patients too. Results The levels of plasma fibrinogen and D-dimer in COPD patients at acute exacerbation stage and remission stage were obviously higher than those in control group, P 〈 0.05. The levels of plasma fibrinogen and D-dimer of COPD patients at acute exacerbation stage were apparently higher than that of remission stage, P 〈 0.05. With the increase of degree of COPD, the levels of plasma fibrinogen and D-dimer gradually increased, the difference was statistically significant ,P 〈 0.05. Conclusion Chronic obstructive pulmonary disease could arouse the raise of the levels of plasma fibrinogen and D-dimer, which may be related to the abnormal coagulation state. The blood system in patients with COPD would be hypercoagulable and its severity positively correlated with the deterioration of COPD.
