摘  要:目的 探讨Orem自护理论在孕妇学校教学模式中应用对提高孕产妇的自护能力、控制胎儿体质量、减少巨大儿的发生率、降低剖宫产率的影响.方法 采用整群抽样方法,以我院2012年1月~2013年2月住院足月单胎孕妇900例为研究对象,其中在门诊接受常规产检、坚持在孕妇学校接受Orem自护理论指导、完成必修课的324例孕妇作为试验组;在门诊接受常规产检,未到孕妇学校接受健康教育的576例孕妇作为对照组,比较两组孕妇分娩的足月新生儿出生体质量、巨大儿发生率、两组产妇的剖宫产率.结果 试验组和对照组巨大儿发生率分别为8.95%、13.54%,P=0.01.两组产妇的剖宫产率分别为52.95%、23.14%,P=0.00.结论 在孕妇学校用Orem自护理论指导下的教学模式进行健康教育,明显提高孕产妇的自护能力,通过孕期营养自我管理,合理的平衡膳食,减少巨大儿的发生率,并有助于降低剖宫产率.Objective The Orem self-care theory used in the teaching model in pregnant women school to improve the pregnant women's self-care ability, and the influence of controling the fetal body quality, reduce macrosomia and cesarean sectionrate. Methods 900 pregnant women in our hospital from January 2012 to February 2013, 324 pregnant women got routine check-ups in the clinic and health education under the guidance of Orem self-care theory in pregnant woman school as the test group, 576 pregnant women only accepted the routine check-ups in clinic, not got the health education in pregnant woman school as the control group, the birth weight of full-term neonates,incidence of macrosomia and cesarean section rate of two group were observed. Results Maerosomia rate of the test group was 8.95% ,while the control group was 13.54%, Cesarean section rate of the test group was 23.14% , while the control group was 52.95% , there were significant difference between two groups. Conclu- sion Health education in pregnant school under the guidance of Orem self-care theory could improve maternal self-care ability, balanced diet, reduce the incidence of maerosomia and contribute to reduce the rate of cesarean section.
