腹腔镜与小切口子宫肌瘤剥除术对机体细胞免疫功能影响的对比研究

摘　　要：目的探讨腹腔镜与小切口子宫肌瘤剥除术对患者机体细胞免疫功能的影响。方法选取腹腔镜及小切口子宫肌瘤剥除术患者100例,其中腹腔镜组49例,小切口组51例。采用流式细胞仪测定患者术前1天及术后第1天、第3天外周血中CD3、CD4+、CD8+及NK细胞表达变化,研究不同手术方式对患者细胞免疫的影响。结果患者术后第1天、腹腔镜组和小切口组CD3、CD4+、CD8+及NK细胞同术前相比表达均明显降低,差异有统计学意义(P<0.05),患者术后第3天腹腔镜和小切口组CD3、CD4+、CD8+及NK细胞同术前相比均基本恢复,差异无统计学意义(P>0.05)。同腹腔镜组比较,小切口组术前及术后CD3、CD4+、CD8+及NK细胞均无统计学意义(P>0.05)。结论腹腔镜与小切口子宫肌瘤剥除术对患者机体细胞免疫功能的影响无显著差异。Objective To study the effect of laparoscopic and minilaparotomy approaches to myomectomy on cellular im- mune function. Methods In the study, a total of 49 patients with uterine myomas received laparoscopic myomectomy,51 patients were through minilaparotomy myomectomy. The T lymphocyte subpopulation were measu red at 1 d before and 1 d and 3 d after opera- tion respectively. The effect on cellular immune function of two groups was evaluated. Results At 1 d after operation,the levels of CD3 ,CIM ~ ,CD8 ＋ and natural killer ce!l（NK） decreased signi ficantly（P 〈 0.05 ） both in laparoscopic group and minilaparotomy group, whereas at 3d after operation those of laparo scopic group and minilaparotomy group changed without statistical significance （P 〉0.05）. There was no significant difference in the levels of CD3,CD4＋ ,CD8＋ and natural killer cell（NK） at ld before and 1 d and 3 d after operation between two groups （P 〉 0. 05 ）. Conclusion The effect of laparoscopic and minilaparotomy approaches to myomect omy on cellular immune function has not significant difference.
