2cm与3cm切缘宽度在乳腺癌保乳手术中的疗效及美容效果比较

摘　　要：目的:探讨2cm与3cm切缘宽度在乳腺癌保乳手术中的疗效及美容效果的比较。方法:入选我院2008年5月至2012年11月收治的64例进行保乳的乳腺癌患者为观察对象,随机分为两组,分别采用2cm和3cm切缘长度,观察两组患者的手术疗效,并对两组患者美容效果评分进行比较。结果:2cm切缘组的危险因素阳性的比例明显高于3cm切缘组(P<0.05),差异具有统计学意义;两组在局部复发率、远处转移率及3年以上生存率方面差异无统计学意义(P>0.05);2cm切缘组的乳房美容优良率明显高于3cm切缘组(P<0.05),差异具有统计学意义。结论:乳腺癌保乳手术行局部扩大切除时,2cm切缘宽度的临床疗效与3cm切缘宽度的效果接近,且术后乳房美容效果更佳,因此切缘2cm宽度已经基本足够,无需进一步扩大切缘宽度。Objective To discuss the comparison between clinical effects and cosmetic results of 2cm and 3cm cutting edge width in breast-conserving operation for breast cancer.Methods 64 patients with breast cancer who chose to recieve breast-conserving operation were chosen and as research objects and divided randomly into 2 groups with 2cm and 3cm cutting edge width separately.Clinical effects and cosmeticresults of both groups were observed and compared.Results Positive rate of dangerous factors of 2cm group was obviously higher than 3cm group （P＜0.05）.The 2 groups were similar in local recurrence rate,distant metastasis rate and more than 3 years survival rate （P＞0.05）.Breast beauty rate of 2cm group was obviously higher than 3cm group （P＜0.05）.Conclusion When doing wide local excision in breast-conserving operation for breast cancer,2 cm cutting edge width is similar to 3cm in clinical effects and it makes the breast look more beautiful after operation.So 2cm cutting edge width is enough,no larger width is needed.
