摘 要:目的探讨胎儿大脑皮层发育异常的产前影像学表现。方法回顾性分析2012年8月至2013年8月接受超声及MRI检查的胎儿280例。结果①280例中诊断为大脑皮层发育异常胎儿12例;②大脑皮层发育异常胎儿中,超声提示诊断经MRI证实8例(小头畸形4例、灰质异位3例、半侧巨脑症1例);超声漏诊后经MRI证实4例(脑裂畸形2例、结节性硬化1例、颞叶发育不良1例);③妊娠进展随访情况为:10例孕妇选择终止妊娠,2例孕妇选择继续妊娠。结论大脑皮层发育异常产前影像表现具有特征性,超声联合MRI对该类疾病产前诊断具有重要作用。Objective To explore the imaging manifestations of fetal malformations of cortical development (MCD). Methods Data of 280 singleton pregnancy women for fetal brain MR1 and ultrasound examination during Aug. 2012 to Aug. 2013 were retrospectively reviewed. Results Altogether 12 cases were diagnosed of MCD, 8 of which were confirmed by MRI after ultrasound findings, and 4 of which were detected after being missed by ultrasound. The 8 cases included heterotopic gray matter ( n = 3 ), microcephaly ( n = 4), and hemimegaiencephaly ( n = 1 ) ; the 4 cases were schizencephaly (n =2), tuberous sclerosis (n = 1 ), and hypoplasia of temporal lobe (n = 1 ). Among these cases, 10 terminated pregnancy, while 2 continued gestation. Conclusion Ultrasound combined with MRI presents typical imaging characteristics, which is a powerful tool to diagnose fetal malformations of cortical development.
