南京31家医院3年肠外营养药物的利用分析

摘　　要：目的：评价南京市区肠外营养（PN）药物利用的情况，为临床营养治疗合理使用和管理提供参考。方法：对南京市区2009～2011年PN药物的零售金额、用药频度（DDD）等指标进行统计分析和评价。结果：南京市区PN药物用量和用药总金额呈上升趋势。近3年用药总金额年均增长率达到21．64％。3年来，用药总金额排位前三名依次为脂肪乳Intralipid、N（2）-L-丙氨酰-L-谷氨酰胺注射液和复方氨基酸注射液。DDD3年排位不变，前三名是复方氨基酸注射液、脂溶性维生素（Ⅱ）和注射用水溶性维生素。作为补充维生素的制剂，脂溶性维生素（Ⅱ）、注射用水溶性维生素存在滥用倾向。结论：南京市区2009—2011年PN药物使用增长幅度较大，在营养药物使用中比例较大，反映临床部分药物存在滥用现象，免疫营养和疾病治疗型营养药物受到临床重视。Objective ： This paper is to assess the utilization of parenteral nutrition （PN） drugs in Nanjing area, and provide references to the proper drugs usage and management of clinical nutrition ther- apy. Methods ： Data of drug sales and DDD in hospitals of Nanjing during the period of 2009 ~ 2011 were analyzed statistically and evaluated. Results： Consumption and dosage of PN drugs increased 36.53% annually in Nanjing area during the past three years, and the top four of consumption were In- tralipid, N （2）-L-Alanyl-L-glutamine Injection, Compound Amino Acid Injection and Fat-soluble Vitamin Injection （ II ）. The rank of DDD unchanged in past three years and the top three were Compound Amino Acid Injection, Fat-soluble Vitamin Injection （ lI ） and Water-soluble Vitamin for Injection. As vitamin preparations, fat soluble vitamins （ I ） and Water-soluble Vitamin for injection had the tendency of drug abuse. Conclusion： The amount of PN drugs increased dramatically in Nanjing area during 2009 ~ 2011, and took a large proportion among the nutrition drugs. It suggested that some clinical drugs fell in trend of abuse. Immune nutrition and disease treatment nutrition drugs were drew attention by clinical therapy.
