白细胞介素18在慢性结肠血吸虫病肠黏膜中的表达及意义

摘　　要：目的 研究慢性结肠血吸虫病患者结肠黏膜中白细胞介素18（IL-18）的表达情况及与性别、年龄的关系.方法 50例慢性结肠血吸虫病病理标本来源于2008年6月1日至2012年11月30日在大理学院附属医院门诊及住院患者（血吸虫病组）,12例正常者为对照组.所有研究对象行全结肠镜检查,血吸虫病组取病变明显处的结肠黏膜3～5块,经病理确诊为慢性结肠血吸虫病.正常对照组结肠镜检查,经肠镜和病理确诊正常肠黏膜3～5块作为正常对照.所有组织标本常规石蜡包埋,常规切片,苏木精伊红染色法（HE）染色,4 μm厚连续切片.采用免疫组织化学链霉菌抗生物素蛋白过氧化物酶连结法（SP法）检测血吸虫病组及正常对照组结肠黏膜中IL-18的表达情况,每个研究部位选取4个高倍视野（400倍）,采用同济大学千屏影像 HPIA-2000高清晰度病理图文分析系统进行分析.结果 血吸虫病组结肠黏膜中IL-18阳性表达的积分光度、阳性单位高于正常对照组,积分光度2.81±1.81 vs 0.42±0.22（P〈0.01） ;阳性单位24.66±6.77 vs 6.71±6.12（P〈0.01）,血吸虫病组结肠黏膜中IL-18表达平均灰度低于正常对照组,189.27±17.06 vs 222.80±15.04（P〈0.01）,但血吸虫病组结肠黏膜中IL-18表达的积分光度、阳性单位、平均灰度均与性别、年龄无关.结论 慢性结肠血吸虫病患者结肠黏膜中IL-18的表达较正常结肠黏膜升高,但慢性结肠血吸虫病患者结肠黏膜中IL-18的表达与性别、年龄无关.Objective To study interleukin 18 （IL-18） expression in the colonic mucosa of chronic colonic schistosomiasis patients and its relationship with gender and age. Methods Fifty chronic colonic schistosomiasis pathology specimens from Jul 1,2008 to Nov. 30,2012 at the Affiliated Hospital of Dali University outpatient and inpatients,12 normal subjects（control group） were set to study the whole line of colonoscopy colonic lesions. All the subjects were checked with the whole line of colonoscopy, the colon mucosa 3 5 samples were taken in the obvious colonic lesions of the patients with chronic schistosomiasis to get pathological diagnosis as chronic colonic schistosomiasis. 12 cases of normal colonoscopy examination, confirmed by colonoscopy and pathology and normal mucosa often 3-5 samples, served as normal controls. All routine paraffin-embedded tissue samples, conventional biopsy, HE stain, 4 #m thick serial sectioning were conducted. Immunohistochemistry SP method was used to detect IL- 18 expression in the colonic mucosa and its relationship association with gender, age. Four high power fields （400 times） were chosen from each study and analyzed by Tongji University in 1000 with screen images HPIA-2000 high- Graphic Analysis System Definition pathological analysis. Results The integral luminosity of IL-18 in the colonic mucosa of chronic colonic schistosomiasis was more than that of normal control group,2.81±1.81 vs 0.42±0.22（ P〈0.01）. The positive units of II.-18 in the colonic mucosa of chronic colonic schistosomiasis was more than that of normal control group,24. 66±6.77 vs 6.71±6.12（ P〈0.01）. The average gray of IL-18 in the colonic mucosa of chronic colonic schistosomiasis was less than the normal control group,189.27±17.06 vs 222. 80±15.04（ P 〈0.01）. But the expressions of IL-18 were not related with gender and age in the patients with chronic colonic schistosomiasis.
