摘 要:目的研究吡格列酮对2型糖尿病患者血清脂联素、TNF-α生物学活性的影响,探讨2型糖尿病与炎症、脂肪因子的关系。方法选取84例2型糖尿病患者为病例组,102例健康体检者为对照组,病例组患者接受吡格列酮治疗3个月,用放射免疫药盒测定血清脂联素,MTT比色法检测TNF-α的生物学活性。结果病例组患者血清脂联素低于对照组(P<0.05),TNF-α的生物学活性高于对照组(P<0.05),吡格列酮治疗后,2型糖尿病患者血清脂联素升高(P<0.05),TNF-α的生物学活性下降,但无统计学意义(P>0.05),血清脂联素与BMI及Homa-IR呈负相关,TNF-α的生物学活性与BMI及Homa-IR呈正相关。结论 2型糖尿病是慢性炎性疾病,炎症状态与肥胖及血糖控制有关,吡格列酮通过增加血清脂联素水平改善胰岛素抵抗。To study the effect of pioglitazone on serum adiponectin level and biological activity of serum TNF-α in T2DM patients. To investigate correlations between T2DM and inflammation,adipokines. Methods 102 healthy vol- unteers served as control subjects, 84 diagnosed type 2 diabetic patients had been treated with pioglitazone for 3 months. The biological activity of serum TNF-α with MTT colorimetric method and adiponectin level with RIA had been measured before and after pioglitazone,respectively. Correlation analysis was conducted between these data and BMI,Homa-IR. Results Decreased adiponectin was found in type 2 diabetes patients compared to the healthy subjects(P〈0.05),T2DM group showed the higher of biological activity of serum TNF-α than that of healthy subjects(P〈0.05). Treatment with pioglitazone elevated adiponectin(P 0.05)and decreased biological activity of TNF-α in type 2 diabetes patients, but no significant difference(P〈0.05). Correla- tion analysis indicated significant negative correlations of serum adiponectin level with BMI and Homa-IR. There was significant positive correlation between biological activity of TNF-α and BMI,Homa-IR. Conclusion T2DM is a low-grade chronic in- flammatory disease, obesity and quality of glucose control may be positively associated with the chronic inflammation. Pioglita- zone which increased adiponectin expression and secretion can effectively improve insulin resistance in type 2 diabetes.
