521例腹腔镜胆囊切除术中腹腔镜探查体会

摘　　要：目的：探讨腹腔镜胆囊切除术中腹腔镜探查的价值。方法研究近3年来本院进行的521例腹腔镜胆囊切除术病例，在术中均进行详细的腹腔镜探查，并对结果进行统计分析。结果除胆囊本身病变外，探查出腹腔内其他病患79例，探查阳性率为15.16%。其中男性阳性率为10.68%（22/206）；女性患者探查阳性率为18.10%（57/315）。男女探查阳性率有统计学差异（χ2＝5.324，P〈0.05）。将所有患者按年龄分组，39岁以下探查阳性率为7.14%（7/98），40~59岁年龄组的患病率为16.23%（37/228），60岁以上年龄组患病率为17.95%（35/195），39岁以下年龄组患病率与其他两组统计学意义（χ2值分别为4.846和6.202，P〈0.05）。结论腹腔镜胆囊切除术中仔细的探查十分必要。且女性患者探查阳性率大于男性患者，40岁以上患者探查阳性率大于39岁以下患者。Objective To discuss the value of laparoscopic exploration in laparoscopic cholecystectomy. Methods The clinical da-ta of 521 cases of laparoscopic cholecystectomy in the last three years were studied. Laparoscopic exploration was carried out during the cholecystectomy, and the results were statistically analyzed. Results The exploration discovered 79 cases of other types of diseases be-sides gallbladder lesions, and the positive rate of exploration was 15.16%. The positive rate of exploration in male patients was 10.68%（22/206）, and that in female was 18.10%（57/315）. There was a significant difference in the positive rates between the male and female （χ2=5.324, P〈0.05）. All the patients were divided into three groups according to their ages. The positive rates of those under 39 years old, between 40 and 59 years old, and above 60 years old group were 7.14%（7/98）, 16.23%（37/228）, and 17.95%（35/195）, respectively. The positive rate of under 39 years old group was lower than those of other two groups （χ2=4.846 and 6.202, P〈0.05）. Conclusion Careful exploration during the laparoscopic cholecystectomy is quite necessary. The positive rate of exploration in the female is higher than that in the male, and that in patients above 40 years old is higher than that in those under 39 years old.
