阴阳调衡透刺法对中风后痉挛瘫患者行走功能重建的影响

摘　　要：目的观察阴阳调衡透刺法对中风后痉挛瘫患者行走功能重建的调节作用。方法将90例中风痉挛瘫患者随机分为治疗组和对照组,每组45例。治疗组采用阴阳调衡透刺法治疗,对照组采用康复治疗。观察两组治疗前后下肢Fugl-Meyer运动功能、Berg平衡功能两项量表评分及步态分析检测。结果两组治疗后Fugl-Meyer运动功能评分及Berg平衡量表评分与同组治疗前比较,差异均具有统计学意义(P<0.01)。治疗组治疗后Fugl-Meyer运动功能评分及Berg平衡量表评分与对照组比较,差异均具有统计学意义(P<0.01,P<0.05)。两组治疗后步态分析各项指标与同组治疗前比较,差异均具有统计学意义(P<0.01)。治疗组治疗后患肢步长、患肢步速、患肢步频、预承重期时限、摆动期时限与对照组比较,差异均具有统计学意义(P<0.01,P<0.05)。结论阴阳调衡透刺针法可有效缓解中风后痉挛瘫患者下肢异常肌张力增高,调节髋膝踝关节活动度,有效改善患者下肢功能。Objective To investigate the regulating effect of yin-yang-balancing point-to-point acupuncture on walking function reconstruction in patients with post-stroke spastic paralysis. Methods Ninety patients with post-stroke spastic paralysis were randomly allocated to treatment and control groups, 45 cases each. The treatment group received yin-yang-balancing point-to-point acupuncture and the control group, rehabilitation therapy. Lower limb function was scored using the Fugl-Meyer Motor Scale and the Berg Balance Scale and gait analysis was made in the two groups before and after treatment. Results There were statistically significant pre-/post-treatment differences in the Fugl-Meyer Motor Scale score and the Berg Balance Scale score in the two groups （P〈0.01）. There were statistically significant post-treatment differences in the Fugl-Meyer Motor Scale score and the Berg Balance Scale score between the treatment and control groups （P〈0.01,P〈0.05）. There were statistically significant pre-/post-treatment differences in indices from gait analysis in the two groups （P〈 0.01）. There were statistically significant post-treatment differences in affected leg step length, velocity, cadence, loading response and swing phase between the treatment and control groups （P〈0.01,P〈 0.05）. Conclusion Yin-yang-balancing point-to-point acupuncture can effectively reduce lower limb hypermyotonia, regulate the activities of hip, knee and ankle joints and improve lower limb function in patients with post-stroke spastic paralysis.
