微型骨锚在急性近指间关节侧副韧带起点或止点损伤治疗中的应用

摘　　要：目的：采用微型骨锚修复急性闭合性近指间关节侧副韧带起点或止点损伤，观察术后疗效并评价其治疗效果。方法：选择14例15指急性闭合性近指间关节侧副韧带起点或止点损伤患者，采用切开、微型骨锚植入重建断裂侧副韧带起点或止点的方法进行治疗。术后2周拆除石膏，进行保护性功能练习，复查患者患指侧方应力试验及关节活动度（ROM），观察指体有无肿胀、畸形和疼痛，x线有无骨锚松动、脱落，采用Dargan功能评定标准评估疗效。结果：术后患者均得到随访，随访时间为6～15个月，平均为10．7个月。所有患者指间关节被动活动稳定、无疼痛，关节ROM70°～110°，平均91°；Dargan评定，优12例，良2例，可1例，优良率93．33％。结论：采用微型骨锚治疗急性闭合性近指间关节侧副韧带起点或止点损伤的方法简便易行，疗效满意。Objective To observe and evaluate the clinical effects of mitck mini anchor in treatment for acute closed starting ending point injury in latera! Cbllateral＇ligament of interphalangeal joint. Methods 14 patients （15 fingers） with acute closed starting or ending point injury in lateral collateral ligament of interphalangeal joint were treated by operation and anchor planted in phalanx to rebuild its starting or ending points. Closed-plaster method was used for 2 weeks after the operation before the protective function practice. Lateral stress test, ROM, tumentia, deformity, pain and X-ray were observed when the patients were rechecked. Results All the patients were followed up for 6 to 15 months, averaged for 10.7 months. The interphalangeal joint of the patients was stable and had no pain and tumentia, and the ROM was 70°--110°, averaged for91°. According .to Dargan function evaluation standard, 12 fingers were excellent, 2 fingers were good, 1 finger was fair, and the fineness rate was 93.33°. Conclusion The method of reconstruction of mitck mini anchor in treal.ment for acute closed starting or ending point injury in lateral collateral ligament of interphalangeal joint is easy and has satisfied efficacy.
