心肌特异性miR-133a与cTnT早期诊断急性心肌梗死的价值

摘　　要：目的 比较血循环中心肌特异性miR-133a与肌钙蛋白T（cTnT）在急性心肌梗死（AMI）早期诊断中的价值.方法 收集67例AMI患者和32名非AMI对照者的血浆标本,通过定量逆转录聚合酶链反应（qRT-PCR）测定血浆中miR-133a水平,同时通过电化学发光法检测血浆cTnT的变化.结果 AMI患者血浆中miR-133a水平较非AMI对照者明显升高（P〈0.01）,而当这些AMI患者康复出院时miR-133a水平已回落到基线.MiR-133a水平的波动与研究对象的自身特征无相关性（P＆gt;0.05）.受试者工作特征曲线（ROC）分析表明,miR-133a在早期诊断AMI方面具有显著的心肌特异性和敏感性,但并不优于cTnT.结论 血浆miR-133a可作为AMI早期诊断的标志物,但在诊断价值上并不优于cTnT.Objective To evaluate the expression of plasma cardiac-specific miR-133a in acute myocardial infarction（AMI） and compare their diagnostic values with that of cardiac treponin T（cTnT）. Methods Sixtyseven plasma samples obtained from patients with AMI and thirty-two plasma specimens collected from non-AMI volunteers were analyzed in this study. The level of cardiac-specific miR-133a was detected by quantitative reverse transcription-polymerase chain reaction （qRT-PCR）, and the level of plasma cTnT was detected using electro- chemiluminescence-based methods on the Elecsys 2010 Immunoassay Analyzer. Results The level of plasma miR-133a was significantly increased in AMI patients （P〈0.01） compared with non-AMI volunteers. The expres- sion of the cardiac-specific miR-133a in AMI patients decreased to close to the baseline at the time of hospital discharge（P〉0.05）. There was no correlation between the level of circulating miR-133a and the clinical character- istics of the study populatian（P〉0.05 ）. Furthermore, receiver operating characteristic（ROC ） curve analyses showed that miR-133a was cardiac-specific and sensitive for the diagnosis of early AMI, but not superior to cTnT. Conclusion The present study showed that the plasma miR-133a may be useful biomarker for AMI but is not superior to eTnT for the diagnosis of AMI.
