双管喉罩联合椎管内麻醉在骨科下肢手术麻醉的临床研究

摘　　要：目的研究临床中骨科下肢手术的麻醉方法,主要是双管喉罩联合椎管内麻醉的应用价值。方法在排除以下情况:腰椎手术后、凝血功能异常者、血小板功能异常者、脊柱有畸形和无法配合的患者,随机将骨科下肢手术的90例患者分成三组,每组30例。A组选择复合全麻主要采用单纯气管内插管静吸,B组选择椎管内麻醉主要采用双管喉罩联合,术中保留患者的自主呼吸,使用吸入七氟醚来全麻,C组仅使用椎管内麻醉。并对比手术过程中三组患者的各项生命体征及不良反应的不同。结果双管喉罩联合椎管内麻醉合并吸入七氟醚,保留了患者的自主呼吸。结论双管喉罩联合椎管内麻醉具有术中生命体征平稳,术后并发症少等优点,非常适合临床骨科下肢手术的麻醉和手术后的镇痛,值得在临床上推广。Objective To study the clinical orthopedics lower extremity anesthesia method, mainly is the double tube laryngeal mask combined spinal canal anesthesia applied value. Methods In the following situation： excluded after lumbar spine surgery, blood coagulation function was abnormal, platelet function was abnormal, spinal deformity, and not be able to cooperate with patients, randomly to orthopedics lower limbs of 90 patients were divided into three groups, each group 30 cases. Choose compound anesthesia group A mainly adopts pure static absorption of an endotracheal tube, group B choose intra-spinal canal anesthesia mainly adopts double pipe joint, A laryngeal mask technique in patients with spontaneous breathing, use sevoflurane inhaled anesthesia, group C use only within the spinal canal anesthesia. Comparing with operation in the process of all kinds of vital signs and adverse reaction of three groups of patients. Results Double pipe joint spinal canal laryngeal mask anesthesia combined inhaled sevoflurane .retained in patients with spontaneous breathing. Conclusion Double pipe joint spinal canal laryngeal mask anesthesia with intraoperative vital signs stable, less postoperative complications, very suitable for clinical orthopedics lower limbs surgery, anesthesia and postoperative analgesia is worth popularizing in clinic.
