血清胱抑素C水平联合尿微量清蛋白/尿肌酐比值测定对妊娠糖尿病患者早期肾功能评价的意义

摘　　要：目的：探讨血清胱抑素C（CysC）联合尿微量清蛋白（UmAlb）、尿肌酐（UCr）比值检测对诊断妊娠糖尿病（GDM）患者早期肾功能评价的价值。方法将92例GMD患者根据糖化血红蛋白（HbA1c）水平将其分为 HbA1c＜6．5％组（A组）34例，6．5％～8．5％组（B组）32例，＞8．5％（C组）26例，并选取同期30例在该院行产前检查的健康孕妇作为对照组，对比分析各组孕妇空腹血糖（FPG）、餐后2 h 血糖（2hPG）、1，3-β-D-葡聚糖（BG）、CysC、UmAlb 和空腹胰岛素（FIns）、餐后2 h 胰岛素（2hIns）、UCr、尿素氮（BUN）、血清肌酐（SCr）的水平，并对 UmAlb/UCr 及 CysC 联合检测的临床价值进行分析。结果 GDM 患者组FPG、2hPG、FIns、2hIns、BG、HbA1c、CysC、UmAlb/UCr、BUN、SCr这些检测指标均高于对照组，差异有统计学意义（P＜0．05）；C组各糖代谢指标及肾功能指标均高于A、B组，差异有统计学意义（P＜0．05）。C 组 CysC 联合 UmAlb/UCr 检测阳性率高于A、B组，差异有统计学意义（P＜0．05）。结论对GDM患者早期肾功能损伤者应用UmAlb/UCr联合CysC检测能有效判断患者肾功能病变情况，两者联合检测能提高诊断的准确度及灵敏度。Objective To investigate the clinical value of combinational detection of serum cystatin C（CysC） and urinary albumin （UmA1b） / urine creatinine （UCr） in the diagnosis of gestational diabetes mellitus （GDM）. Methods 92 GMD patients were divid- ed into three groups accorded to glycosylated hemoglobin （HbA1c） level： HbA1c 〈6.5% （A group） 34 cases,6.5%-8.5% （B group） 32 cases,〉8.5 % （C group） 26 eases. Thirty normal pregnant women were selected as control. The levels of FPG,2hPG, BG, CysC, Fins, 2hIns, UCr, BUN, SCr were detected and compared. Then the clinical value of UmAlb/UCr and CysC were ana- lyzed. Results The levels of FPG, 2hPG, Fins, 2hlns, BG, HhA1 c, UmAlb/UCr, BUN, SCr of GDM were significantly higher than control group. The index of group C were higher than group A and B （P〉0.05）. The positive rates of LTmAlb/UCr and CysC were significant higher than group A and B （P〈0.05）. Conclusion Combination detection of UmAlb / UCr joint CysC can help judge the renal function of the GDM patients with early renal injury.
