摘  要:目的探讨超声三维造影成像技术对颈动脉粥样斑块内新生血管的检测及临床意义。方法对伴有颈动脉粥样软斑块73例患者按照临床有无脑梗死分为脑梗死组(32例)和非脑梗死组(41例),对其颈动脉粥样软斑块行超声三维造影成像检查,按统一标准测量并记录各斑块体积及斑块内新生血管指数(VI),作对比分析。结果脑梗死组颈动脉软斑块的血管指数明显高于非脑梗死组(P=0.023),而两组患者斑块体积、最大厚度及颈动脉管腔的狭窄率差异均无统计学意义。结论超声三维造影成像技术可对颈动脉粥样软斑块内的新生血管行定量检测,为研究斑块内的微血管灌注提供了一种新方法。Objective To explore the quantitatively assessment of the neovascularization within carotid soft plaques in patients with or without cerebral infarction using contrast-enhanced three dimensional ultrasonic imaging and its clinical value. Methods A total of 73 patients with carotid soft plaques were divided into two groups: Group A including 32 patients with cerebral infarction; group B including 41 patients without cerebral infarction. The contrast-enhanced three dimensional ultrasonic imaging was used to examine each soft plaque of the two groups of patients. The volume and number of the neovascularization of every soft plaque was recorded by unitive standard. The result was analysed to compare if there was difference about the vascularity index ( VI ) between the two groups of soft plaques. Results The VI of the soft plaques between the two groups were significantly different ( P=0.023 ) ; but the volume, the largest thickness and the stenosis rate of the carotid between the two groups were not significantly different ( P〉0.05 ) . Conclusions The neovascularization within carotid soft plaques can be quantitatively evaluated using the contrast-enhanced three dimensional ultrasonic imaging, which provide a new method to investigate the microvascular perfusion of the soft plaques.
