肾血管平滑肌脂肪瘤破裂出血的介入治疗

摘　　要：目的：探讨肾血管平滑肌脂肪瘤破裂出血行超选择性肾动脉栓塞的介入治疗。方法37例患者均行数字减影血管造影（DSA），明确诊断后行超选择性肾动脉栓塞术。结果术后所有患者24h后腰区胀痛明显减轻，48-72 h 后血尿逐渐消失，术后未出现明显并发症，预后良好。结论肾血管平滑肌脂肪瘤通过DSA血管造影及栓塞治疗后可以迅速达到满意的治疗效果，能有效地控制出血并能最大限度保护正常肾组织。Objective To assess the interventional therapy of super-selective renal artery embolization in raptured renal angiomyolipoma. Methods All 37 patients of raptured renal angiomyolipoma underwent digital subtraction angiography（DSA）. After diagnosis,Patients were given super-selective renal artery embolization. Results After super-selective renal artery embolization,the waist area pain were reduced in all patients significantly after 24h,gradual disappearance of hematuria were seen 48~72 h after the DSA.No obvious complications occurred,the prognosis was good. Conclusion DSA angiography and embolization can quickly reach a satisfactory therapeutic effect for rapture renal angiomyolipoma,which can effectively control the bleeding while maximize the protection of normal kidney tissue.
