摘  要:目的 观察载阿仑膦酸钠(ALN)的磷酸钙骨水泥(CPC)、磷酸钙骨水泥、同种异体骨修复兔股骨骨缺损的效果.方法 取健康成年新西兰大白兔36只,制备直径4 mm,深10 mm的双侧股骨髁部缺损动物模型,随机分为3组(n=12),于缺损处植入载ALN的CPC(A组)、CPC(B组)、同种异体骨(C组).术后3、6个月取标本行生物力学测试和Van-Gieson染色行组织学观察.结果 生物力学测试结果:缺损部位3个月A组为(39.54±4.40) mPa,B组为(31.66±6.12) mPa,C组为(18.83 ±3.35) mPa;6个月A组为(45.01±3.87) mPa,B组为(33.47±5.46) mPa,C组为(23.07±3.51) mPa,差异有统计学意义(P<0.05);股骨中段3个月A组(25.18 ±5.54) mPa,B组为(16.05 ±3.61) mPa,C组为(15.35 ±4.27) mPa;6个月A组为(31.38 ±4.96) mPa,B组为(16.84±3.87) mPa,C组为(16.61 ±4.07) mPa,3、6个月股骨中段部位压缩强度A组与B、C组比较差异有统计学意义(P<0.05),B、C组间差异无统计学意义(P>0.05).组织学观察显示,3个月,A、B、C3组材料周围有不同程度的新生骨生成,A、B组材料有不同程度降解,新生骨与材料结合,C组新生骨与材料界限明显,6个月,3组新生骨量增加,A、B组材料降解明显,新生骨沿降解孔隙向内部生长,A组优于B组,C组材料无明显变化.组织学定量分析:3个月骨组织含量A组为(27.66±4.28)%,B组为(15.23±4.61)%,C组为(6.34±5.82)%;6个月A组为(36.35±5.71)%,B组为(19.37±5.60)%,C组为(10.67±4.49)%,3、6个月新生骨组织含量百分比A组>B组>C组(P<0.05),各组内新生骨量6个月优于3个月.结论 载ALN的CPC加快修复兔股骨骨缺损,并增强股骨强度.Objective To investigate the effectiveness of calcium phosphate cement (CPC) loaded with alendronate (ALN),CPC or allografts bone to repair rabbit femoral bone defects.Methods The bilateral femoral condylar defects models (4 mm in diameter,and 10 mm in depth) were established in 36 adult and healthy New Zealand rabbits,and randomly divided into 3 groups (n =12).CPC as drug delivery system loaded with ALN (group A),CPC (group B),or allografts bone (group C) was implanted in the defects.At 3rd and 6th month after operation,the specimens were collected for mechanical test and histological examination.Results Biomechanical testing results showed in groups A,B and C was (39.54 ±4.40),(31.66 ±6.12) and (18.83 ± 3.35) mPa at 3rd month of the defects,and that was (45.01 ± 3.87),(33.47 ± 5.46) and (23.07 ± 3.51) mPa at 6th month of the defects,respectively.There was statistically significant difference when the 3 groups were compared in pairs.The midshaft of femur in groups A,B and C was (25.18 ±5.54),(16.05 ±3.61) and (15.35 ±4.27) mPa at 3rd month of the defects,and that was (31.38 ±4.96),(16.84 ±3.87) and (16.61 ±4.07) mPa at 6th month of the defects,respectively.The compressive strength of the midshaft of femur in group A was increased significantly as compared with groups B and C (P 〈 0.05),and no significant difference was found between groups B and C (P 〉0.05).Histological examination results revealed that at 3rd month after operation,there were different degrees of new bone tissue regeneration in group A,B and C,the material in groups A and B partly degraded,and new bone tissue bound with the material tighter in groups A and B than in group C.At 6th month,the materials had further degradation in the groups A and B,and new bone tissue grew into the gap.The bone grew better in group A than in group B,and there was no significant change in group C.The quantitative analysis results demonstrated in groups A,B and C was �
