初产妇产后抑郁的发病情况及其影响因素的研究

摘　　要：目的 调查初产妇产后抑郁的发病情况并探讨其影响因素.方法 研究对象为2012年10月~2013年8月在我院住院分娩的足月、产后6周的710例初产妇,产后抑郁情况采用Edinburgh产后抑郁量表（EPDS）进行调查统计,影响因素包括一般资料及产科因素.结果 710例初产妇中123例患者有不同程度的抑郁.一般资料中高学历、公务员、教师及医护职业、居住条件满意的患者的抑郁发生率较低,差异有统计学意义（P＜0.05）;产科因素中无意外妊娠、无妊娠合并疾病、顺产、新生儿性别为男性的初产妇抑郁发生率较低,差异有统计学意义（P＜0.05）;多因素Logistic回归分析结果表明学历、意外妊娠、妊娠合并疾病、新生儿性别、分娩方式均为初产妇产后抑郁发生的影响因素（P＜0.05）.结论 初产妇产后抑郁症的发生受到多种因素的影响,应引起医务工作者及产妇家属的重视.Objective To investigate the incidence of postpartum depression and explore the influencing factors. Methods The subjects included 710 cases of early maternal postpartum, all of them were full-term birth, partum six weeks and selected from October 2012 to August 2013 in our hospital. The depression of all the patients were surveyed with Edinburgh Postnatal Depression Scale circumstances （EPDS）, influencing factors included general information and obstetric factors. Results Among 710 cases of primipara,there wore 123 patients with varying degrees of depression. The depression incidence of the highly educated, civil servants, teachers and health care occupations, living Conditions satisfying in the general information were lower, with significant difference （P〈0.05）; the depression incidence of no unwanted pregnancies, no pregnancy sickness, birth, newborn child sex to male in the obstetric factors were lower, with significant difference （P〈0.05）;multivariate Logistic regression analysis showed that the education, unwanted pregnancy, disease, pregnancy, newborn gender, mode of delivery were primipara the risk factors of postpartum depression （P〈0.05）. Conclusion Early postpartum depression occurs is affected by many factors, medical workers and mothers families should mention.
