综合ICU医院感染目标性监测分析

摘　　要：目的了解综合重症监护病房(ICU)医院感染特点、评价目标性监测效果,为持续质量改进和提高ICU医院感染管理水平提供科学依据。方法采用目标性监测方法,收集2010年10月-2012年9月综合ICU 2319例患者发生医院感染及危险因素资料,采用Excel电子表格录入数据资料,然后对数据资料进行描述性统计分析。结果综合ICU患者发生医院感染感染率为3.02%,例次感染率为4.14%,日感染率为12.61‰,患者日例次感染率为17.29‰,平均病情严重程度为2.91分,调整感染率为1.04%,调整例次感染率为1.42%,调整日感染率为4.33‰,调整日例次感染率为5.94‰;医院感染以下呼吸道感染居首,呼吸机相关性肺部感染发病率为38.80‰;医院感染病原菌以革兰阴性杆菌为主,占69.45%。结论综合ICU医院感染以下呼吸道感染为主,呼吸机相关性肺部感染的监测与控制措施有待进一步加强。OBJECTIVE To understand the characteristics of nosocomia infections in general ICU, and to evaluate the effect of targeted monitoring, so as to provide scientific basis for constant improvement of medical quality and level of nosocomial infection management. METHODS A targeted monitoring method was adopted to collect the data of nosoeomial infection and risk factors of 2319 patients in a general ICU from Oct. 2010 to Sep. 2012, and used Excel spreadsheet was used to input data, and then a descriptive statistical analysis was conducted on these data. RESULTS The rate of nosocomial infection in general ICU of our hospital was 3.02%, case-time infection rate was 4.14 % ~ the nosocomial infection incidence per patient-day was 12.61‰, case-time infection incidence per patient-day was 17. 29‰; the score of disease severity in ICU patients was 2. 91. After the adjustment, the incidence of nosocomial infection was 1. 04%, case-time infection rate was 1. 42%, the nosocomial infection incidence per patient-day was 4.3‰, and the case-time infection incidence per patient-day was 5.94‰. The most infection site was lower respiratory tract, and the constituent ratio of ventilator associated pulmonary infection reached 38.80‰. The main nosoeomial infection pathogens were gram-negative bacilli, which accounted for 69. 45%. CONCLUSION The most infection site of nosocomial infection is lower respiratory tract in our general ICU, and the monitoring and control measures should be strengthened for the ventilator associated pulmonary infection.
