中西医结合治疗对重型脑外伤恢复期患者的疗效分析

摘　　要：目的 探讨中西医结合治疗对重型脑外伤恢复期患者的临床疗效.方法 选择2011年10月～2012年12月入住我院脑外科治疗重型脑外伤恢复期的患者76例,按就诊顺序分为两组.对照组给予高压氧治疗,观察组在对照组基础上给予针灸联合穴位注射纳洛酮治疗,两组患者均治疗2个疗程（60d）后比较两组患者偏瘫运动功能、日常生活能力、认知功能改善情况.结果 观察组和对照组治疗后手部、上肢、下肢的运动功能与治疗前比较均有显著改善（P ＜0.01）,生活自理能力、制约括约肌能力、转移能力、运动能力、交流能力、社会认知能力等各项FIM评分和总分与治疗前比较均有显著提高（P ＜0.01）,认知功能MMSE评分与治疗前比较有显著提高（P ＜0.01）,各项数据比较观察组改善情况均明显优于对照组（P ＜0.01）.结论采用针灸配合穴位注射纳洛酮的中医手段联合西药和高压氧治疗重型脑外伤恢复期患者,可以有效改善运动功能、生活自理能力,提高认知功能,有效提高患者的生存质量.Objective To investigate the clinical effect of combined treatment of traditional Chinese medicine and western medicine in patients with severe brain injury in the recovery stage. Methods Seventy-six patients with severe brain injury in the recovery stage admitted into the cerebral surgery department of our hospital from October 2011 to December 2012 were selected, and randomly divided into the two groups. The patients in the control group were given hyperbaric oxygen therapy, while the patients in the observation group were given acupuncture combined with acupoint injection of naloxone therapy based on the control group. The improvement on motor function of hemiplegia, activities of daily life and cognitive function assessment of patients in the two groups were compared after two treatment courses（60 days）. Results Compared with before the treatment, the patients in the two groups had more significant improvement on the hand, upper limb and lower limb motor function, FIM score such as living self-care ability, ability to restrict the sphincter, transfer ability, athletic ability, communication ability, social cognitive ability and total score, and MMSE score of cognitive function after the treatment （all P 〈0. 01 ）. While the indexes improvement of the observation group were significantly better than those of the control group （P 〈0. 01）. Conclusion Acupuncture combined with acupoint injection of naloxone as a combined treatment of traditional Chinese medicine and western medicine method and hyperbaric oxygen in the treatment of patients with severe brain injury in the recovery stage can effectively improve the motor function, living self-care ability, cognitive function and life quality of patients.
