摘 要:目的:分析讨论生化检验对糖尿病肾病早期的临床诊断。方法随机选取我院2012年1月至2012年12月的72例糖尿病肾病患者为糖尿病组,同期选择72例健康体检者为对照组,对两组分别测定生化指标 a1-MG(a1微球蛋白)、UalB / Ucr(尿白蛋白/尿肌酐)、rBP(视黄醇结合蛋白)、CRP(c 反应蛋白)、cysc(血胱抑素 c)、U-malB(尿微量清蛋白),比较两组的差异性。结果对照组的 a1-MG、UalB / Ucr、rBP、crP、cysc、U-malB 的检测结果分别为5.62±1.48 mg/l 、3.45±0.19、39.7±12.7 mg/l、1.21±0.38 mg/l、0.71±0.23 mg/l、10.75±6.38 mg/l,糖尿病组的各检测结果分别为36.51±4.57 mg/l 、59.85±0.67、84.1±17.4 mg/l 、10.32±3.28 mg/l 、1.23±0.29 mg/l 、41.39±15.42 mg/l,糖尿病组的各测定结果均高与对照组,两组相比较具有显著性差异;糖尿病组的 a1-MG、UalB/ Ucr、rBP、crP、cysc、 U-malB 检测的阳性率分别为:61.1%、44.4%、54.1%、34.7%、59.7%、51.2%,六项联合检测的阳性率为94.4%。结论联合检测各生化指标对糖尿病肾病患者的早期诊断、对肾脏损害的程度等均具有重大的临床意义,值得在临床推广应用。Objective discussion biochemical test for clinical diagnosis of early diabetic nephropathy. Result control group a1-MG, UALB / UCr, RBP, CRP, CysC , U-mALB results were 5.62 ± 1.48 mg/L, 3.45 ± 0.19, 39.7 ± 12.7 mg/L, 1.21 ± 0.38 mg/L, 0.71 ± 0.23 mg/L, 10.75±6.38 mg/L,the detection results of diabetic group were 36.51 ± 4.57 mg/L, 59.85 ± 0.67, 84.1 ± 17.4 mg/L mg/L, 10.32 ± 3.28, 1.23 ± 0.29 mg/L, 41.39±15.42 mg/L, the determination results of diabetic group were higher compared with the control group, compared the two groups had significant difference; the positive rate of diabetes group of a1-MG, UALB / UCr, RBP, CRP, CysC, U-mALB detection were: 61.1%, 44.4%, 54.1%, 34.7%, 59.7%, 51.2%,positive rate of combined detection of six is 94.4%. Conclusion the combined detection of various biochemical markers for early diagnosis of diabetic nephropathy in patients with renal damage degree, to have significant clinical implications, it is worth in clinical application.
