长春西汀对老年冠心病患者血浆TNF-α、MCP-1、IL-18及血液流变学的影响

摘　　要：目的:观察长春西汀治疗老年冠心病的疗效及其对炎症因子TNF-α(肿瘤坏死因子-α)、MCP-1(单核细胞趋化蛋白-1)、IL-18(白细胞介素-18)及血液流变学的影响。方法:将100例老年冠心病患者随机分为对照组50例,给予常规药物治疗;治疗组50例,在常规药物治疗的基础上运用30mg长春西汀。9天为一疗程。结果:①治疗组与对照组比较,总有效率具有显著性差异,P<O.05。②两组治疗后TNF-α、MCP-1、IL-18水平均较前下降,且治疗组改善明显优于对照组,差异具有统计学意义(P<O.05)。③两组治疗后红细胞压积、纤维蛋白原、全血黏度高切、全血黏度低切、血浆黏度均较治疗前降低,且治疗组疗效明显高于对照组,差异具有统计学意义(P均<0.05)。结论:①长春西汀能够提高老年冠心病的临床疗效。②常规药物治疗的基础上加用长春西汀,在降低炎症因子TNF-α、MCP-1、IL-18水平及改变血液流变学的特点上有一定意义。Objective：To observe the efficacy of vinpocetine to treat coronary in elderly patients and its impact on the inflammatory cytokines TNF-α （TNF-α）, MCF-I （monocyte chemoattractant protein-1 ）, IL-18 （ intedeukin- 18 ） and hemorheology. Methods： 100 cases of coronacy were randomly divided into a control group of 50 patients were treated with conventional therapy ;treatment group of 50 patieats were treated with 30rag vinpocetine after tile conventional therapy. 9 days for a course. Resuits：（1）Treatment group compared with the control group, the total effective rate was significant differ- ence （ P 〈 O. 05 ）. （2）After treatment, TNF-α, MCP-1, IL-I 8 levels decreased than before treatment, and the treatment group decrcased significantly better than the control group, the difference was statistically significant （ P 〈 O. 05 ）. （3）After treat- ment, hematocrit ,fibrinogen, high and low shear of whole blood viscosity, plasma viscosity were lower than before treatment, and the efficacy of treatment group was significantly higher,the difference was statistically significant （P 〈0.05 ）. Conclu- sion ：（1）Vinpocetine can improve the clinical efficacy of coronary in elderly patients. （2）Vinpocetine [roses on the convention- al therapy in reducing inflammatory cytokines TNF-α, MCP-1, IL-18 levels and changes the ci,araeterisfics of blood dleology has some significance.
