益肾胶囊对糖尿病肾病模型肾小管间质TLR4及NF-κB表达的研究

摘　　要：目的观察益肾胶囊对糖尿病肾病(DN)大鼠模型肾小管间质中炎症因子Toll样受体4(TLR4)、核因子-κB(NF-κB)表达水平的影响。方法 48只健康雄性Wistar大鼠随机分成正常对照组、DN模型组、苯那普利组及益肾胶囊组,每组12只,利用单侧肾切除加腹腔注射链脲佐菌素(STZ)建立DN大鼠模型,连续给药12周。取肾组织行HE染色,观察病理组织学变化;采用免疫组织化学染色法检测各组大鼠肾组织中TLR4及NF-κB表达水平的变化。两组间检测数据比较采用独立样本t检验;多组间检测数据比较,方差齐时采用单因素方差分析,方差不齐时采用非参数秩和检验。结果造模12周时模型组大鼠部分肾小管上皮细胞空泡变性,肾小管扩张,间质炎症细胞数增加,益肾胶囊组大鼠肾小管上皮细胞的病变减轻。模型组大鼠肾小管间质中TLR4及NF-κB表达水平高于正常对照组,差异有统计学意义(t=3.50,2.37;P<0.05)。益肾胶囊组大鼠肾组织TLR4及NF-κB表达水平明显低于模型组,差异有统计学意义(t=3.71,3.56;P<0.05)。结论益肾胶囊可能通过调节肾组织炎症因子TLR4及NF-κB的表达,减少DN的炎症反应,从而延缓DN的进展。Objective To investigate the effect of Yishen capsule on expression of toll-like receptor 4 （TLR4） and nuclear faetor-KB （NF-KB） in the tubulointerstitium of rats with experimental diabetic nephropathy. Metbords Forty-eight Wistar rats were randomly divided into four groups： normal control group, diabetic nephropathy （DN） control group, benazepril treatment group, and Yishen capsule treatment group. DN was induced by combination of right nephrectomy and peritoneal injection of streptozotocin （50 mg/kg）. The rats in benazepril treatment group and Yishen eapsule treatment group were treated with benazepril （10 mg/kg） and Yishen capsule （625 mg/kg） by garage for 12 weeks, respectively. The tubulointerstitial histological changes were investigated by light microscopy observation of hematoxylin-eosin （HE） staining renal tissues. Expression levels of TLR4 and NF-KB in tubulointerstitium of the rats were detected by immunohistochemical staining. The differences between two groups were compared with independent sample t test, while differences among multiple groups were compared with single-factor chi-square test if variance was homogeneous, or with nonparametric rank sum test if variance was heterogeneous. A difference was considered statistically significant when P value was less than 0.05. Results At twelve weeks after the establishment of diabetic nephropathy model in rats, some renal intersitia showed vaeuolar degeneration in tubular epithelial cells, tubular dilatation, and an increase of interstitial inflammatory cells, which were ameliorated in Yishen capsule treatment group. The expression levels of TLR4 and NF-KB were significantly higher in DN control group than in normal control group （t = 3.50, 2. 37 ; P 〈 0.05 ） , but were significantly lower in Yishen capsule treatment group than in DN control group （ t = 3.71, 3. 56 ; P 〈 0.05 ）.
