超声引导下神经根阻滞对低位神经根型颈椎病患者的疗效观察

摘　　要：目的:评价超声引导下神经根阻滞治疗低位神经根型颈椎病患者的临床疗效。方法:选取低位神经根型颈椎病患者60例,采用随机数字表法,将患者随机分为两组,每组30例,超声引导颈神经根阻滞治疗组(U组,n=30)与非超声引导颈神经根阻滞治疗组(N组,n=30)。记录治疗前、治疗后30 min、1、2、4和8周疼痛视觉模拟评分(visual analogue scale,VAS);记录准确穿刺例数,神经根注射/阻滞区域吻合例数,治疗次数≥2的例数;记录治疗后4、8周疗效评定及不良反应发生率。结果:与治疗前相比,两组治疗后各时点VAS<4的例数明显增多。治疗后30 min和1周,U组VAS<4的例数明显多于N组(P<0.05);U组准确穿刺例数明显多于N组,神经根注射/阻滞区域吻合例数,U组明显多于N组,U组治疗次数≥2的例数明显少于N组(P<0.05);两组治疗后4、8周疗效评定及不良反应发生率,差异无统计学意义。结论:超声引导下神经根阻滞治疗准确率高且治疗次数少,是一种可视化、安全、可靠的治疗技术。Objective： To evaluate the effects of ultrasound-guided nerve root block on lower cervical radiculopathy.Methods： A total of 60 patients with lower cervical radiculopathy were randomly divided into2 groups： the ultrasound group（group U,n = 30） and the non-ultrasound group（group N,n = 30）.Visual analogue scale（VAS） was recorded before treatment and 30 minutes,1 week,2 weeks,4 weeks,and 8 weeks after treatment.The number of cases of accurate injection and cases with consistency of nerve root injection site and block area,cases with treatment times ≥ 2 were also recorded.Effcacy evaluation and incidence of adverse reactions were also recorded at 4 and 8 weeks after treatment.Results： The cases with VAS 4 were increased at each time points after treatment in two groups.Compared with group N,there were signifcantly more cases with VAS 4 in group U at 30 minutes and 1 week after treatment（P 0.05）.Compared with group N,there were more cases of accurate injection and more cases with consistency of nerve root injection site and block area in group U,while there were less cases with treatment times ≥ 2 in group U（P 0.05）.There was no significant difference in efficacy evaluation and incidence of adverse reactions between two groups.Conclusion： The ultrasound-guided nerve root injection has high accuracy and requires less treatment times,which is a safe and reliable visualized therapeutic method.
