口服药物联合肋间神经脉冲射频治疗胸背部带状疱疹后神经痛的疗效观察

摘　　要：目的:观察口服药物联合肋间神经脉冲射频治疗胸背部带状疱疹后神经痛的临床效果。方法:选择胸背部带状疱疹后神经痛患者62例,采用随机数字表法随机分为对照组30例和治疗组32例。对照组采用口服药物治疗,治疗组在口服药物治疗的基础上,联合在肋角处对肋间神经行脉冲射频治疗。记录两组治疗前、治疗后第1、7、30、60天的视觉模拟评分(visual analogue scale,VAS)、睡眠质量评分(quality of sleep,QS)和曲马多用量,并观察治疗相关的并发症。结果:与治疗前相比,治疗组在治疗后第1、7、30和60天,VAS和QS明显降低,曲马多用量也明显减少(P<0.05);在治疗后的第1、7、30和60天,治疗组的VAS、QS和曲马多用量均明显低于对照组(P<0.05);治疗组未见治疗相关的并发症。结论:口服药物联合肋间神经脉冲射频治疗胸背部带状疱疹后神经痛,能明显减轻患者疼痛和减少镇痛药物的使用剂量,操作简单安全,有临床应用价值。Objective： To observe the clinical efficiency of oral medication combined with intercostal nerve pulsed radiofrequency for postherpetic neuralgia（PHN） in thoracic back region.Methods： Sixty-two patients with PHN in thoracic back region were randomly divided into control group（n = 30） and treatment group（n = 32） by using random digits table.Patients in control group were treated with oral medication and patients in treatment group were treated with intercostal nerve pulsed radiofrequency at angulus costae combined with oral medication.Visual analogue scale（VAS）,quality of sleep（QS） and tramadol dosage before and 1 d,7 d,30 d and 60 d after treatment,and complications during the treatment were recorded.Results： Compared with before treatment,the VAS and QS of the treatment group decreased signifcantly at 1 d,7 d,30 d and 60 d after treatment,and tramadol dosage also reduced obviously（P 0.05）.The VAS,QS and tramadol dosage of the treatment group at 1 d,7 d,30 d and 60 d after treatment were obviously lower than those of the control group at the same time points（P 0.05）.No serious complications occurred in the treatment group.Conclusion： The therapy of oral medication combined with intercostal nerve pulsed radiofrequency for PHN in thoracic back region can signifcantly relieve the pain of patients and reduce the dosage of analgesic drugs.The operation is simple and safe,which has clinical application value.
