摘 要:疼痛是机体炎症、创伤等时的伴随症状,一般分为急性伤害性疼痛、炎症性疼痛和神经病理性疼痛,一定程度上有赖于外周初级感觉传入神经元的激活。疼痛与免疫系统密切相关,组织损伤伴随着受侵袭部位或固有免疫细胞释放炎症介质:细胞因子、炎症趋化因子、神经生长因子、ATP和前列腺素类引起痛觉过敏和痛觉超敏。同时,炎症诱导受损组织周围的免疫系统释放出阿片肽与阿片受体结合抑制神经元的兴奋性发挥免疫镇痛作用。该综述总结了阿片受体-阿片肽参与外周镇痛的免疫机制,并讨论了治疗疼痛的外周免疫镇痛机制的新研究靶点。Pain which is accompanied by symptoms of inflammation,trauma,generally divided into acute nociceptive pain,inflammatory pain and neuropathic pain,depends on the activation in the peripheral afferent neurons.Pain is closely related to the immune system,which is characterized by increased sensitivity to mechanical or heat stimuli of the affected tissue.Following tissue injury,invading or resident immune cells release inflammatory mediators,including cytokines,chemokines,nerve growth factor,ATP and prostaglandins.Meanwhile,opioid peptides released by inflammation-induced immune system of damaged tissue bind to opioid receptors causing analgesia by inhibiting the excitability of neurons.This review summarizes the immune mechanism of opioid receptors-opioid peptides in the unique endogenous peripheral analgesic pathway and discusses therapeutic implications for the development of novel targeted peripheral analgesics.
